There are 23 Hexes. Four are found within the content that was DLC on the Xbox 360 version of the game.
Two are in the Sunken King area which you'll need the Dragon Talon to access. It can be found on a corpse behind a locked door near the bottom of the Majula pit. Take Gilligan's longest ladder down, then a shorter ladder up to the door, which requires the forgotten key from the Giants in the Black Gulch.
The access point for this content is just beyond the Rotten's primal bonfire.
Promised Walk of Peace is on a corpse in a cave close to the Tower of Prayer bonfire:
Dark Greatsword is in a chest close to the Priestess' Chamber bonfire:
One is in the Ivory King area which you'll need the Frozen Flower to access. It can be found on a corpse in Drangleic Castle in the a room just beyond the large room with six doors immediately after the King's Gate bonfire. It's in plain sight in front of a Ruin Sentinel statue.

Dark Dance is on a corpse on a hill near the Abandoned Dwelling bonfire:
One is in the Old Iron King area which you'll need the Heavy Iron Key to access. It can be found in the salamander area behind a locked door just before the Last Giant (in the Forest of the Fallen Giants). You need the Iron Key from the Iron Keep to unlock this door.
The access point for this content is just beyond the Old Iron King's primal bonfire.
Recollection is in a corpse at the bottom of a tower most easily accessed from the Foyer bonfire:
The remaining 19 Hexes can be found or bought/traded from NPCs.
Felkin the Outcast is in a cave in Hunstman's Copse just before the Undead Refuge bonfire. You must have 8 Intelligence and 8 Faith to interact fully with him.
Straid of Olaphis is petrified at the top level of a tower just before the Sinner's Rise bonfire in the Lost Bastille. You must unpetrify him with a Fragrant Branch of Yore.
Darkgiver Grendahl is the leader of the Pilgrims of Dark covenant. You must talk to him in the Shaded Woods, the Black Gulch, and Castle Drangleic before you can interact fully with him.
Stone Trader Chloanne is initially encountered just past the Poison Pool bonfire in Harvest Valley. After exhausting her dialogue, she will move to Majula.
Cromwell the Pardoner is in a room above the cathedral where the Prowling Magus is fought in Brightstone Cove Tseldora.
Magerold of Lanfir is found in the Iron Keep. Turn right and follow the hallway once you enter the main building.

Promised Walk of Peace - Crown of the Sunken King DLC. Just before second bonfire are some red eyed statues you can destroy to reveal a pillar, hit that to move another platform, use this to enter a cave and grab the spell.
Dark Greatsword - Crown of the Sunken King DLC. Activate the pillar near the third bonfire here, and ride the activated elevator halfway down then jump onto a ledge then head outside over a small platform to find this spell.
Recollection - Crown of the Iron King DLC. In the room directly above where you get the Smeltor Sceptre, in one of the small rooms attached, found on a corpse.
Dark Dance - Crown of the Ivory King DLC. Just after second bonfire. Kill the enemies in the first area then head right before climbing the stairs along a snowy cliff to find this spell on a corpse.

Repel - Purchased from Straid of Olaphis with the Giant Lord Soul.
Twisted Barricade - Doors of Pharros. From Gyrm's Respite bonfire, go through the area until you climb a ladder, at the top just to the left will be a corpse with the hex.
Numbness - Purchased from Straid of Olaphis with the Old Dead One's Soul from killing The Rotten on NG+ or using a Bonfire Ascetic.
Scraps of Life - Purchased from Cromwell the Pardoner. Found in The Black Gulch in front of the first worm in a jar down the path from the first bonfire.

Promised Walk of Peace - Shulva Sanctum City. Close to the second bonfire, just before going on the platform up to it you will see some red eye statues, shoot the pillar to rise the platform and enter the cave, the hex is on a corpse next to the tree.
Dark Greatsword - Shulva, Sanctum City. From the Priestess' Chamber bonfire, hit the switch close by to activate the elevator. Ride it down and roll off of it when you see a ledge and follow the path to the left around the building to a chest with the hex.
Recollection - Brume Tower. You will climb down a ladder to a dark room with several doors leading to other rooms. Inside one of these rooms is a corpse holding the hex.
Dark Dance - Frozen Eleum Loyce. In the first area before climbing the stairs near the second bonfire. Go to the right and around a building up a snowy path to a corpse holding the hex.

• Resonant Soul - acquired by bringing devotion to the covenant of the Pilgrims of Darkness (Guardians of the Abyss) to the first level. The covenant is brought to level 1 by clearing one “Void Chasm”, any of the three portals that Grendal opens. When clearing, be sure to light the Fire Cup !
• Great resonant soul Great Resonant Soul) - acquired by bringing devotion to the covenant of the Pilgrims of Darkness (Guardians of the Abyss) to the second level. The covenant is brought to level 2 by clearing all three portals of the Abyssal Chasm. Be sure to light the Fire Cup in everyone!
• Climax - acquired upon receiving the third level of devotion to the covenant of the Pilgrims of Darkness (Guardians of the Abyss). The covenant is brought to level 3 by clearing all three portals of the “Void of the Abyss” and killing the boss Hiding in the Darkness at the end of the last portal! Appears only if all 3 Fire Bowls are lit!

• Complete Silence (Profound Still) - purchased from the leader of the Pilgrims of Darkness covenant (Guardians of the Abyss).
