How to unlock the Supreme Weapon achievement in DARK SOULS II: Scholar of the First Sin - Definitive Guide

To get this achievement you must upgrade a weapon to +10 by reinforcing it through a blacksmith with titanite.

You need a total of 6 titanite shards, 6 large titanite shards, 6 titanite chunks, and a titanite slab. Also, each upgrade costs souls, which depends on the weapon and level.

+3 - takes 6 titanite shards to this point.
+6 - takes 6 large titanite shards to this point.
+9 - takes 6 titanite chunks to this point.
+10 - takes one titanite slab.

You can reinforce your weapon at Blacksmith Lenigrast in Majula, Steady Hand McDuff in the Lost Bastille, and Weaponsmith Ornifex in Brightston Cove Tseldora.

Titanite shards are easy to come by, particularly as items found in lower level areas, can be bought from Lenigrast, and Chloanne has an unlimited amount of them to sell.

Large titanite shards are easy to buy, McDuff sells an unlimited amount, as does Chloanne. They are also dropped by various enemies such as Old Ironclads (turtle guys) in the Forest of Fallen Giants.

Titanite chunks are more difficult, Chloanne sells an unlimited amount after the final boss is defeated. They are also a rare drop from Gyrm Warriors in the Doors of Pharros. The fastest way to get them is to join the Bell Keepers as you receive one with each successful invasion.

Titanite slabs are very rare, but not quite as bad as in the first game. Chloanne sells one (late-game), another is inherited from McDuff, they can be given by Dyna and Tillo in Things Betwixt, and one from the Pharros contraption underneath the ballista room in the Forest of Fallen Giants. Another is dropped by the dragonrider in the Undead Crypt. You have a chance to receive a titanite slab when defeating an invading Bell Keeper grey spirit.

The Bell Keeper covenant also gives a chance for a titanite slab for defeating a host (instead of a chunk), and you also receive one for reaching rank 1 (10 kills). Stone statues near the King's Gate bonfire in Drangleic Castle may drop slabs, allowing to farm slabs. Thanks to Catatonic Nali for this information.

I also just received a slab for reaching Rank 1 in the Bell Keepers. So after 10 successful invasions go talk to one of the dwarves and he'll give you a slab.

Updated the guide with your information, and confirmed that you can get slabs from defeating hosts as a Bell Keeper and that you get one for reaching covenant rank 1 (Bell Keepers).

Titanite chunks can also be farmed from the worms that are in the oil pits in black gulch, they are rare but you can get them there

Achievement not unlocking, reinforced to max. On x1 achievement app, progress is 100 percent and it says underneath "done! unlocking..." and this was done with the black knight axe +10 and Smelter hammer +10 and infused raw. WTF?

Don't worry about this too much. I also had an achievement that did the same (Rat King covenant). It unlocked after about a week. Some people have theirs unlock in a day or two. Others have been known to take a month. You should just ignore it for now, it will eventually unlock.

Does a +5 also work when that is the max? Like Staff of Wisdom?

I've just reached the Drangleic Castle area with the ruin sentinels and stone statue knights and I can confirm that the stone statue knights definitely drop slabs and their drop rate seems fairly high. I'm not using any gear that increases drop rate and I've already received 2 slabs off their corpses after killing about 15 of them. Assuming they don't despawn after 12 kills this will again be a fantastic place to farm titanite slabs.

I just received a slab after a successful Bell invasion instead of the usual chunk. Not sure if it was because it was my 5th win overall, because I'd won a couple in a row or if it's purely random.

Back on the 360 version last year I found that the statue knights in Drangleic Castle drop slabs as well but this might not be true in SotFS. There are a bunch of them in the large room just by the King's Gate bonfire. They don't despawn after you've killed them 12 times making this one of the only places in the game you can farm enemies for souls. The drawback is the room also contains 5 Ruin Sentinels so you need to despawn all of them to farm effectively which means killing 60 Ruin Sentinels.
But once you've cleared them out you can run round the room killing all the statue knights really quickly and they occasionally drop slabs so in theory this is an unlimited supply of slabs.

To get this achievement you will need to reinforce a standard weapon to +10 or a special weapon to + 5 at 1 of the 3 blacksmiths in the game: Lenigrast in Majula, Steady Hand McDuff in the Lost Bastille, and Weaponsmith Ornifex in Brightston Cove Tseldora. To get a standard weapon to +10 you will need to a total of 6 titanite shards, 6 large titanite shards, 6 titanite chunks, and a titanite slab. Each weapon also costs a certain number of souls to upgrade.

You will need the following to max out a regular weapon: 6 Titanite Shards, 6 Large Titanite Shards, 6 Titanite Chunks, 1 Titanite Slab (most of these are found through natural progression, a slab can be found in the Lockstone room below the Ballista room of Forest of Fallen Giants). Plus, the required amount of souls to upgrade the weapon. The easiest blacksmith to find is Lenigrast in Majula. Speak to him and buy his shop key from the merchant by the Cardinal Tower bonfire, then head back and open the store. Once you upgrade a weapon to +10 with the above items, this will unlock.

For this trophy, you need to reinforce any standard weapon to +10 or special weapon to +5 using Petrified Dragon Bones or Twinkling Titanite. I recommend reinforcing a standard weapon as it is much quicker and easier. The Fire Longsword you can find in The Forest of the Fallen Giants is an excellent choice. You should find plenty of materials playing the game naturally, but if not, Stone Trader Chloanne will sell everything that you need once you have defeated the final boss. You will need the following:

6 Titanite Shards
6 Large Titantite Shards
6 Titanite Chunks
1 Titanite Slab

There is a girl in the Harvest Valley location, she sells fragments, pieces, etc. It is better to improve the Two-Handed Sword because it will increase 400 damage by +10 pumping

You need to download weapons that are convenient to use. For me, Large Club is much nicer than Great Sword. The damage to different creatures is different. Heavy armor penetrates better with Blunt than with Slash. But it’s better to hit the naked ones with Slash or Pierce. Magicians pump out their junk, clerics pump out theirs.
The fastest way for me to collect materials is to run through the Harvest Valley and the Forgotten Fortress, therefore to Brightson-Tseldora Bay. I started with a guide - how to get the Agape ring at the minimum level or stay forever young.
