Of course you can put some more effort into customizing the look of your character, but that's completely optional.

You will get this trophy after speaking with the old ladies at the beginning of the game.

Once you start the game, you will wake up in a field and move towards a house up ahead. Do not attack anything yet. Once there, you will speak to some old Firekeepers and be able to choose you name and character. This will unlock once you are done.
