The Estus flask can be upgraded for more uses by Andre in Firelink. Upgrades require Estus Shards, which are found in various locations throughout the world.
You start the game with three uses in your Estus Flask. The first item you find is the Ashen Estus flask (which has one use). You can toggle the distribution of uses between the Estus flasks at Blacksmith Andre in Firelink Shrine. There are a total of 15 uses to be had if you find all 11 Estus Shards.
1) Proceed from the Tower on the Wall bonfire in High Wall of Lothric out onto a roof and then down a ladder. You can enter a building from this platform and eventually reach a large room with several enemies- melee guys and dogs. The Estus shard is on an anvil:
2) Proceed from the Undead Settlement bonfire through a building and into an open area with numerous enemies. Several will be gathered around a tree with a fire burning at its base. The shard is in plain view at the base of this tree:
3) Not far from the Crucifixion Woods bonfire in Road of Sacrifices, head between two stone buildings and under an arch of sorts. You'll see a fire burning on the ground. Head left and the shard will be on a corpse:
4) Close to the Cleansing Chapel bonfire. The shard is on a corpse at the base of a monument where four enemies are praying:
5) From Farron Keep bonfire in the Road of Sacrifices, look out and slightly left across the swamp and you'll see a broken bridge and an item under an arch, which is the shard on a corpse:
6) From the Anor Londo bonfire in Irithyll of the Boreal Valley, head up the stairs and straight ahead into the building. The shard is in a chest in a dimly lit area along the left wall:
7) From the Old King's Antechamber bonfire in the Catacombs of Carthus, there is an illusory wall across the room. Break it and take your first left (watch the slimes on the ceiling) and the shard is on a corpse at the end of the hallway:
8) From the Irithyll Dungeon bonfire, go across a bridge to the other side of the dungeon and downstairs, then outside to a giant open area. The path leads to the right and down some steps. In the next room is a chest- a mimic- that contains the shard:
NOTE: throw an Undead Hunter Charm at the mimic before you attack and it will not go hostile to you.
9) From the Dancer of Boreal Valley bonfire in High Wall of Lothric, go up the ladder and turn to the left. Continue along until you reach a lift that leads to the Consumed King's Garden. Jump off of halfway down, take the path to the right, and the shard is on a corpse:
10) From the Grand Archives bonfire in Lothric Castle, take the lift up and head out towards in the direction of the Prince Lothric boss area. As soon as you get outside, turn left and head up some stairs, circling around onto the roof where three big flying Knights will engage you. This circular roof has several exits that connect to a path around the perimeter, but one exit leads to an enclosed area where the shard is on a corpse:
11) Buy the 20,000 souls key from the Handmaid in Firelink that can be used to unlock the door at the back/right side of Firelink. Head up to the bridge and drop off the left side onto the roof. At the front of the building, drop down a level and go inside. The shard is on a corpse on a rafter:
Note: the first 10 are listed largely in discovery order, but this last shard can be gotten as soon as you can afford to purchase the key from the Handmaid.
On an aside: I didn't track Estus shard locations on my first playthrough, where I was very diligent with exploration and actually found them all. I decided that during my 2nd playthrough I would find them again and record the clips above, and when I reached the end of my playthrough, I had found 11 and thought that I was missing four. Since the flask maxes out at 15, I just assumed that there were 15 shards to be found. I couldn't understand how I had missed four of them and went crazy for a few hours scouring the world in the hopes of finding shards that didn't even exist. Then I remembered that you start with three uses in your flask and thought I was missing one shard, so I went crazy again looking for the one that I thought I had missed. I finally realized that the first item you pick up (the Ashen Estus flask) gives you a charge and that there are only 11 shards to be found.

You start with a flask and four shots of Estus (one is assigned to your Ashen Flask) but need to find eleven more to max out your flask. Here is the order you should grab them in:
1) In the High Wall of Lothric head to the Tower on the Wall bonfire, go down the steps and out of the door ahead onto a rooftop. Down the ladder here and then down the ladder at the other end of the roof. At the bottom follow the path straight ahead until you reach a large room with the shard on an anvil.
2) In the Undead Settlement go through the first large building and at the bottom you'll come out into an open area with a bridge on the right and plenty of enemies around a bonfire on the left. The shard is in front of the bonfire.
3) Buy the Tower Key for 20,000 souls from the Handmaid in Firelink, then head outside the arches on the top level of the shrine and head left to open the door. Head up to the next roof where the birds nest is and you can drop down to a ledge below, find an opening here to enter the Shrine raftters to find this item on a corpse.
4) As you go past the Crucifixion Woods bonfire in Road of Sacrifices, head to the building ahead but go down the left side. You'll come to a ledge looking down on a bonfire with two large hollows, the shard is on a corpse down here.
5) Right outside the door of the Cleansing Chapel bonfire. The shard is on a corpse surrounded by four praying enemies.
6) From Farron Keep bonfire head forward and slightly left to a broken bridge to find this on the other side near a toppled tower.
7) From the Old King's Antechamber bonfire in Smouldering Lake go and hit an illusionary wall in the far right corner. Go through here and look down the left corridor, there are a bunch of slimes on the ceiling so kill them and grab this from the floor at the end.
8) From the Anor Londo bonfire go up the stairs then left at the top and into the room with the dead giant, go up the stairs in here and follow the path down killing deacons on the way. When you get to main floor at the bottom face away from the boss room and check along the right side for a chest with this shard.
9) From the Irithyll Dungeon bonfire head straight and then left, cross the bridge here and left at the other side to find stairs leading down. Head along the downstairs and outside to some large steps. Follow them down past the enemies to a room with a chest - IT'S A MIMIC, so kill it to get the shard.
10) After you kill the Dancer of Boreal Valley, head up the ladder and then left to the stairs, kill the knight and take the lift but jump to a ledge halfway down. Grab the shard from a corpse on the ledge outside.
11) In the Grand Archives climb all the way to the highest roof, where three large Gold Winged Knights drop down to attack you. If you go to the right side of this big roof area (as you enter) you'll find the shard on a corpse.

You don’t have to worry too much about finding Shards. In the new game+ you will get the missing shards for the maximum level.
1. Fire Temple - use the Tower Key to enter the tower located behind the Fire Temple. Climb the stairs and go across the bridge. Approximately in the middle of the bridge, you can jump first onto the bridge support, then onto the roof of the temple. Run along the roof to the end and jump down to the ledge below. There will be an entrance under the roof of the fire temple. The fragment lies on one of the intersecting beams, directly under the chick's nest. If you use the tree method, you can get this fragment one of the first in the game.
2. High Wall of Lothric - after the second bonfire, go to the hollow with the black parasite, go down the stairs to the Lothric Knight with the spear and go along the corridor. Next there will be a hall with a dog, several hollows with axes and a hollow throwing bombs from above. In this room there is a fragment on the altar.
3. Settlement of the undead - on a corpse near a burning tree, near which a preacher stands and peasants pray.
4. Path of Sacrifices - the building where the hollow magician is first encountered. Go through it to the bridge. On the left there is an area with burning wood and two opponents (they are quite strong, be careful). From them, go to the end of the area, there will be a corpse with a fragment.
5. Farron Citadel - from the fire of the same name to the left to the burning log. Nearby there will be a bunch of slugs and a corpse with a fragment on it.
6. Temple of the Depths - to the left of the entrance to the chapel to the second bonfire, on the memorial with the praying hollows.
7. Smoldering Lake - bonfire The Old King's Hallway. There is an illusory wall in the corner of the room with the fire. We turn left, running through the corridor.
8. Garden of the Consumed King - from the Cold Valley Dancer bonfire, go up the stairs and turn left. We go down the elevator and, before reaching the end, we jump from it onto the platform. There will be a corpse with a fragment on the right.
9. Anor Londo - area in front of Aldrik with many slugs and deacons. The chest with the fragment is located behind the right stairs (if you stand with your back to the fog).
10. Irithyll Dungeon - bridge with crossbowmen, in the next room there is a chest. Be careful, this is a mimic, from which a fragment falls.
11. Lothric Castle - on the roof of the Great Archive, after the three angels.
The maximum number of flasks is 15 (4 starting + 11 fragments).
When upgrading the last estus - achika may not fall out. The reason is unknown.
Make backups of your saves before the last upgrade.
Otherwise, you can only get it when playing as a new character.
