My first solution, I was worried how it would be received.

You can farm both large titanite shards and green titanite shards from the slimes in the The Depths, there's a bonfire a room away from where they load so you can kill, reset, kill, reset. With an item stat of 300 I averaged 1 drop of either every 2 runs (if you add a little more to your runs there's 10+ rats you can farm as well for humanity) a fire weapon will speed up your runs (slime weakness).

Yuhans Item giving 300 item discovery that Comyre mentioned ( Covetous Gold Serpent Ring) can be found in Sen's fortress. This is a qoute from Dark Souls Wiki: "Get to the area with the boulder directing device and move the lever so that it goes west (same way as proper way to go to continue upstairs). let the boulder roll 5-6 times so it feels up the hole then move the lever North (empty space). Now go into the West path and follow it through to the end. You will find a ring on a corpse in a window and a way into a room where the firs MIMIC chest is."

Leeches in Blighttown drop 5 green titanite shards at once if I'm not mistaken:)

For large titanite shard I recommend fighting big knight with a shield in Sen's Fortress (one that's worth 700 souls). He's close to the bonfire and drops large titanite shards quite often even with 100 item discovery. If you have 300 item discovery he drops it more often than not:)

large titanite shard's can also be bought at Sen's Fortress, farm at darkroot garden. Humanity can be bought also from the woman behind bars in the depths.

Just thought I would mention another good place to farm, in Blighttown across the swamp, opposite end of Quelaags domain. There are leech things everywhere. These guys drop large shards, and lots of 5 green titanite shards also. With the serpent ring you get a ton.

You do need the rusty iron ring from Undead Asylum to traverse the swamp though.

Titanite shard is 800 at the blacksmith, large one is 4000 at Sen's Fortress. @ Sceneman, ring does help alot although poison doesn't have to kill you.

There's also a sneaky little trick you can use to reduce the amount of farming you'll have to do throughout the game. It's the easiest (if not the only!) way to get all the weapon enhancement achievements in a single playthrough of the game. The game WILL save any ascensions to the weapon, but not the further enhancements, so long as you quit out to the dashboard as soon as the achievement message fades. When you reload, you'll have retained any materials since the most recent time the weapon was ascended (so, for this one it'll come back as a standard Raw weapon, and you'll keep the extra Titanite Shards that you'd "spent" to get it to +1->+5, so you can spend those on other weapons later!)

Just to mention if you buy the weapon upgrade kit from the blacksmith, you can upgrade your weapon at any bonfire once you've ascended it....

Just wanna say that you actually need a total of 10 large Titanite Shards, cause of the one you need to transform the weapon into raw.

Yes, I was about to post what Freezer said. Andre requires a large shard to actually ascend the item to Raw, which makes ten required in total.
