How to unlock the Ring the Bell (Quelaag's Domain) achievement in Dark Souls - Definitive Guide

To be able to even ring the bell you need to find Quelaag's Domain.

Preparation

Firstly, starting from the bonfire in the tunnel (the second bonfire in Blighttown), you'll want to become human. I also highly recommend that you kindle the fire.

Why? because a few steps away from the bonfire, a NPC Phantom will attack you.
She is pretty easy to kill, you shouldn't have a problem, just keep your shield up and attack after she's taken a swing.

After killing her, you get her meat cleaver, BUT MORE IMPORTANTLY, you will now be able to summon her to assist you in the battle against Quelaag.

Finding Quelaag

Now, actually getting to Quelaag. She isn't too far away, but the issue is getting across the swamp.
Do you see that white lump in the distance? That's where she is.
Sure, you can go all the way round taking 10 minutes and using all your Estus after the golems crush you repeatedly.
OR you can dash across, sticking to the right, only facing 3 big bugs.
Sure, you'll get poisoned. But in the grand scheme of things, this will have very little effect. It'll run out before too long.

When you get to the beach area, and enter the little cavern, abstain from attacking what appears to be 2 guys with some nasty-ass eggs on their backs. They will not attack, so long as you don't.

You will see the summon sign for the NPC you defeated earlier. Her purpose will be to divert Quelaag's attention and attacks from you, helping considerably.

Attacks (courtesy of the Dark Souls Wiki)

Slashes: She slashes either left to right or right to left. Shield or roll to avoid these. Not a huge threat.
Stab: This attack has much more range that you expect it to. Can be a significant threat.
Lava: When she pats the spiders head or the spider looks like he is going to vomit she will use this attack. She will spit lava on the ground around her, if you are far enough away she may shoot 4-5 balls of lava at you.
AoE Explosion: Whenever she reaches down the sides of the spiders head she is prepping her AoE attack. Back up keeping your shield toward her. Large Threat!!

Strategy

The core strategy for fighting Quelaag is to stay to her front sides, waiting for her to spew lava so that you have time to do some damage.
She dramatically telegraphs her movies so after a few attempts there are not many moves that will surprise you.
Use the Black Knight Shield, or some other high fire resist shield, and fire resistance/physical resistance armor as this helps stop much of the damage from her fire AoE attack.

Rewards!

You will recieve the Soul of Quelaag. Don't use it, as with the help of a blacksmith you can re-create her sword with either a Katana or a Scimitar!
oh, you also get a nice 20,000 souls too.

Once you've done with her, you simply go up the stairs, pull the lever and net yourself 15 gamerscore!

Looks like someone didn't read your request about why they gave you a negative. I don't understand why, this is a solid strategy, the exact same one that I used myself (before I read your guide, anyway; I'm giving you thumbs up 'cause great minds think alike :) ).

What I liked best was what you said about reaching her domain, about getting poisoned; it's so true. Depending on your level, getting poisoned doesn't do THAT much harm in the end (you have to use one Estus Flask at least before the battle), and it removes the panic of trying NOT to be poisoned. Simply kill three cragspiders and a couple of pairs of mosquitoes, then wait for it to wear off and you're set.

And I didn't know about the Katana/Scimitar being used to make her sword later! Definitely hanging on to that soul.

Good guide. Only fools shall negate it.

Summoning the NPC might not be so useful as you might think, I get the feeling she got more health if you summon the NPC.

@lord verh: How can you say summoning the NPC isn't useful? I only got hit a few times, and it was my first attempt, going in completely blind (no idea what the boss was, or how to fight it).

Oh, and in terms of the guide, it may be worth adding that the bonfire you're referring to is in Blight Town. ;)

Great guide, I would also recommend checking for a human phantom to help you as well. I've had decent luck finding summon marks near the bonfire so check around there after you go human.
You can summon the NPC phantom and a human phantom to make the fight that much easier. Just remember the phantoms can't heal without you healing, so if you see the NPC low make sure to heal yourself and they will be healed as well.

I had Mildred trying to run back through the white gate the first two times I summoned her in. She died without ever hitting the damn spider angry I also played stupidly, forgetting to change all my poison resistant build gear to fire resistant and such.

If being poisoned hurts a low level significantly, use a purple moss item from the tree enemies in the forest to cure yourself (and possibly not waste an estus flask)

Oh man I thought that NPC was an actual player. I ran away. Now I have to find more humanity to summon her again. :/

How do you tell the difference between an NPC and an actual player invading you?

Press Y to hold your weapon in both hands. Just do it on your own. No need to summon NPC. Walk behind her at the start and slash her nether regions. Always stay close and use B to roll out the way when she attack then go back in and whack her backside again. Always make sure you press Y before the fight!

CrackforBrumak's solution is spot on, but in regards to strats you really do not have to do much if Mildred is there.

The Maneater known as Mildred will take 99% of Quelaag's attention - all I did was hang back and shoot her with arrows. Mildred is a tank, so most of Quelaag's attacks, although deadly to you, on your first playthrough will be nothing to your friendly NPC. wink

Let Mildred take the lead, and just keep your distance. If you have magic, either from the sorcerer tree or the miracle tree, then you can get this done faster. Pryomancy does not work as well (nor should it - she is fire! laugh), and arrows will take a bit longer, but they still work.

The second bell can be found in the depths of Queelag's Domain, which can be reached by going deeper and deeper underground.

Start by taking the right-hand path from Firelink Shrine up to Undead Burg and from there you'll advance to Undead Parish, probably beating the Taurus Demon boss on the way. From there one of the possible paths is to go to the lower part of the city where you'll find the Capra Demon boss, who will reward you with the key to the Depths. Use it to open the nearby door to access the Depths area where you'll eventually face the Gaping Dragon boss and gain the key to Blightown.

You'll find the entrance to Blightown in one of the large areas of the sewers near a merchant, take it and keep advancing deeper and deeper until you reach the bottom floor which will be a huge swamp. In one of the sides of this swamp you'll spot a big spider-web like structure where there'll be the passage to finally reach Queelag's Domain, beat the Chaos Witch Quelaag boss and finally ring the second bell.
