How to unlock the Ultimate Bad Ass achievement in Dark Void - Definitive Guide

This isn't really a solution as it is a fair warning: even if you beat the game on hardcore difficulty you are not guaranteed the achievement. Myself and others have played through the game on the proper difficulty only to not receive the achievement. There is no 100% fix for this but some have had it pop a few chapters in by starting another play through.

Of the 3 main achievement-related glitches, this one is the rarest but just be forewarned before playing.

It really sucks, this game wasn't great in the first place so having to redo the whole damn thing is obnoxious and I'll likely never bother.

Can confirm that I'm also here asking if I have to start a new game or if this can be done through chapter select?

If you finish the game on casual then you can use chapter select and play all the chapters on hard-core and the achievement will unlock. This makes it easier then to start a whole new game and you can have all your weapons at level 3, which makes things slightly easier, especially the maxed out jetpack. Overall this isn't too hard, make use of cover, used sniper rifle or Desintegrator grenade launcher, and take your time.

Complete the game on Hardcore difficulty. The game is not very difficult, even on Hardcore. As long as you utilize the cover system and let your health regenerate as needed, you'll be fine. There are a few trouble spots, but overall the difficulty is manageable. This will also unlock the Dark Void trophy. The trophy will unlock during the closing cinematic, after The Tower of Return chapter is completed.

Note: A number of sources have said that if you complete the game on a difficulty other than Hardcore then go back and use chapter select to play through the game on Hardcore, the Ultimate Bad Ass trophy will not unlock. I recommend playing it safe and playing the game on Hardcore to begin with or starting a new game if you play on a different difficulty initially. If you do start a new game, all your upgrades and tech points will transfer over from your previous one.

The game really isn't that hard. Use the Opressor and Grenades if you get bombarded with enemies. Watch out for the Dark Red enemies they blow themselves up and leave a Gravity Grenade which will kill you very quickly. Follow all my instructions for each level listed above and you should have no problem beating this game on Hardcore.
