Each hero will need to take 5 visits to the Shrine of Reflection to unlock their entire backstory(and skills). You do not need to do the memories for the Bounty Hunter for the achievement.
The placement of these is random but you can purchase stagecoach items that help with general scouting or the shrine map. The Librarian(Sprawl lair boss) has a 100% scouting trophy called Unabridged Edition, which is also a huge help.
Two of each hero's memories will have some sort of event that involves you playing the event out, which means there is a possibility to fail. Some on the other hand are a no-win situation and actually require you to lose.
