How to unlock the Bravo Old Chap achievement in Darksiders II - Definitive Guide
As of May 2013 (confirmed in February 2014) you don't have to get over wave 100 in order to fight Wicked K. And believe me you don't need any video guides to beat him. I think that this "boss" is more like an Easter egg rather than actual BOSS. Ready? Off we go!
0. Make sure you do the following steps on your New Game + and you are Level 25 (otherwise you won't have the Death Grip ability, which is necessary to get to WK).
1. Enter the Crucible and complete first 25 waves (it's very easy even on Apocalyptic), then die on wave 26. (Thanks to Evilplyr for confirmation).
2. Quit, then reenter The Crucible. Return back to the arena and answer "I will choose my own path/way" (something like that) when asked. Then on the right there will be stage select and on the left "Fight Wicked K" option magically appeared!
3. Select it, climb some pillars to reach WK (you will need Death Grip to do that) and kill him quickly (on Apocalyptic it took 3 "teleport strikes" to kill him for me). Cheevo pops here!
OPTIONAL: If you still need to complete the Crucible itself and want it fast, just lower the difficulty to EASY, re-enter the Crucible, select the stage you finished on and complete all waves. On easy it will be like walk in the park. Achievement unlocked!
If you feel like Rider of Apocalypse and want it hard way, then stay on Apocalyptic and go all the way up to Stage 4, where some tricky fights await. Anyway believe me, even on highest difficulty it's not that hard. Just be careful and take your time on waves 85+ and make sure you do the Crucible preferably on your second play-through, be Level 25 (the higher the better - basically because of the helpful skills you can learn) at the very least and have scythes with life steal or high level of "life per kill/crit". For all other things just select those which have maximum Crit Damage and Crit Chance.
Hope that this will help you to get one more achievement, which probably be the last one as it was for me, so congrats!
Good luck and thanks for all the fish. :)

This solution is "the best thing to happen since digital watches."
It's good to know that "Young Serge plays it safe."

You can die earlier than stage 4. Thanks to a stupid mistake I died wave 64. I quit and reenter The Crucible and "Fight Wicked K" appeared.
So you can die stage 3 sure and perhaps stage 2.

Wicked K is unlocked after beating the Crucible in one sitting - waves 1-100. You _cannot_ die/leave without completing all of them in one go. See the following:
Solution for Is There Anyone Else? In Darksiders II
for some help with that. You need to be at least level 25 to unlock waves 75-100 so this will have to be done on New Game+ (as there is a level cap of 22 on your first playthrough).
Once you do that, the Arena will open up and you need to climb up to Wicked K (for which you need the Deathgrip, so make sure you have at least grabbed it from the Foundry). You are permitted to leave after beating wave 100 to restock if you wish (just come back, talk to the Arena Master and choose to start on Wicked K from his wave selection list - the middle will rise again).
Climb up and beat him. He is not particularly difficult if you stay away from him (he has some close up moves that hurt). I was able to do it on level 25 with 5 maxed Teleport Slashes and stacked crit chance (on Easy). For some specific info check out the end of the following guide:

Once you beat him you get a scene that makes you think you just took some acid and this nice achievement.

Maybe this is a post-patch thing but I was unable to leave the Crucible after wave 100. I had to go fight Wicked K without being able to restock on potions. Thankfully he isn't that tough.

I am currently a lvl 30, I tried doing the Crucible in one go, but at Wave 75 I get rewarded with a Chest and I can't continue forward to Waves 76-100... Is this a glitch? I have tried this twice, and at wave 75 I can't continue. :(

Requires the Crucible Online Pass
Wicked K is the secret boss found at the end of the Crucible. Wicked K automatically unlocks if you clear waves 1-25, 25-50, 50-75, or 75-100 in one go without quitting (Credit to kakihara301). After you clear one set of the waves you will be able to face Wicked K whenever you want. So do not worry about dying on him and having to repeat a series of 25 waves again to attempt him.
For more information about the Crucible see the Is There Anyone Else? trophy in this guide.

Sometimes there may be a lag that makes it impossible to reach the Wicked K.
I’ll describe it using my own example: I passed the 100th level of “Cornilla” without restarts or deaths, and I think, now I have platinum in my pocket! But when I tried to climb up to the newly formed building, I encountered an insurmountable obstacle - trying to jump from a wooden beam onto a wall covered with roots, Death either fell, refusing to cling to the roots, or was completely unable to get to the other side along the wall.
And here it’s worth saying the main thing that I was leading to: Unholy K is considered a SEPARATE stage of the Gornilla (In the event that all the conditions for the battle with him are met), so you can safely make a save, restart the game, and go fight with a calm soul by this tea lover.

To access the boss, you need to go through all 100 waves of the Crucible, starting from the first, without dying or restarting from the checkpoint. The 101st wave will be the boss, and they give a trophy for him.

It is not necessary to complete the entire crucible to fight Unholy K. I made it to crucible level 74, died, loaded up, and ended up in the “waiting room” in front of the arena. I went to the main one, started a conversation with him and chose the option: “I will choose my own path” and they offered me to fight the wicked.

Small addition: to get to Unholy K. you will definitely need Death Grip.
