An epic conflict with a boss indeed!
Stand back boys and girls, this is an awesome throwdown between our protagionist Death, and the gigantic corrupted Guardian in the field of Stonefather’s Vale.
Phase 1: You start out on Despair, you’re going to want to circle the Guardian clockwise, staying on the outside of his right arm. Once he swings his hammer you’ll at the outside of his forearm with Redemption where you’ll notice clusters of Shadowbombs. Hit these and his arm will blow off, then he’ll take a knee. You need to dismount and target the Maker’s heart on his shoulder with Deathgrip. Pull yourself up and swing away with your scythe. Rinse and repeat a few times dependant on your difficulty level you’re playing at.
Phase 2: This time the Guardian attacks with his left arm which is a giant, rolling Shadowbomb cannon. You’ll want to circle counterclockwise this go round but try to stay in-line with the cannon, when he raises to fire, run towards it so the bomb goes overhead. The bombs hone in on you so while riding Despair, lock on to the bomb chasing you and fire with Redemption until the bomb stops and starts shaking. At this point you need to dash in behind the Guardian putting him between the path of the bomb and yourself. When the bomb strikes it’ll drop him to a knee again. Run around to the front and you’ll need to wall dash up two prongs sticking out and end up perched on a beam, look left towards the Maker’s heart, jump and latch on with Deathgrip then go all "slashy slashy" with your scythe. Again rinse and repeat.
Phase 3: Sit back and watch a bad ass cutscene, weep a little for the fallen and head towards the Tree of Life.
Upon completion you’ll unlock the intended achievement as well as another:

On phase 2, I didn't even bother climbing up the leg. I just ran towards the knee spamming the Deathgrip, and I pulled onto the stone straight from the ground. Dunno if it was a glitch or not, but it saved me a lot of trouble.

This is story related trophy that will unlock naturally during your playthrough of the game. For additional information with potential spoilers, read the detailed description below.
Once you have restored the Guardian during the “Heart of the Mountain” quest chain you will be forced to fight it, and win. After the battle concludes there will be a lengthy cutscene, at the end of which this trophy will unlock simultaneously as the Heart of the Mountain trophy. You will also receive a tome which contains your invitation (the Fool card) to the first stage of the Crucible.
