How to unlock the The Rod of Arafel achievement in Darksiders II - Definitive Guide
Story related, cannot be missed.
You will net this achievment deep into the main questline after you find your way to Lostlight from the Kingdom of the Dead. You will proceed through Lostlight to the Crystal Spire where a portal to the Earth will be opened to you.
Upon arriving at Earth you will speak with Uriel who will point you in the direction of the rod. Follow the very linear areas to collect the 3 pieces of the rod, constantly returning to this main hub after each piece is found (note that each section will feature a fight with the same creature called The Noss. This creature shouldn't give you too much trouble as you will always have an angelic weapon on hand to use should you desire it).
After you collect the third piece, return to Uriel who will speak with you. Once the conversation is complete, proceed to the portal to return to Lostlight. As you exit the portal, the achievement should unlock during the following cutscene.

@OZELU - This game achievements are very glitchy. Apparently Vigil just didn't QC them very well as every guide I've posted has had at least 1 person saying they never got the achievement despite it cannot be missed. Vigil is working on fixes (supposedly) so hopefully you'll get a patch for it someday soon.

This is story related trophy that will unlock naturally during your playthrough of the game. For additional information with potential spoilers, read the detailed description below.
The Archon sends you on a quest to earth to collect the Rod of Arafel from Earth. After collecting the three pieces of shattered rod you assemble it and bring it back to the Crystal Spire. A cutscene will begin with the Archon and the trophy will unlock.

Story trophy. In the third world you will be in a post-apocalyptic city, the task is to get 3 pieces of the rod. You are guaranteed to find them just by walking forward. In this location you can pick up two types of firearms: the angelic machine gun and the hellish stabbing gun. I recommend the hellish hammer, I couldn’t die with it even on maximum difficulty. If a crowd is attacking you, stand still and send the stakes into the crowd or onto the floor - they will explode in a couple of seconds. Along the way you will come across hefty creatures that like to ram, often it is enough to retreat a little back into the corridor from where they came, and fire from there.
