this as many other achievements can be obtained cumulatively in several playthroughs, as long as they are NG+ on the same save slot

Fragments of Adamantine are scattered throughout the levels, found after breaking objects, and can be dropped by enemies after you've defeated them. They can also be bought at serpent holes by Vulgrim for 2,400 souls a piece.
