Word of possible warning. If you change the difficulty mid game, even if you go higher. This achievement may be voided. In my experience, I changed it higher, then back to normal without coming out of the menu and I did not receive this upon completion.

In order to get this achievement, select Normal difficulty or higher and remain on that difficulty for the entirety of the game.

Can confirm. I just switch a difficulty in the menu by accident and didn't leave the menu. switched it back to normal and didn't get the cheevo

Here to confirm two things:
i) Upping the difficult will void all other difficulty specific achievements except the one for completing the game on ANY difficulty.
ii) Beating NG+ aka Armageddon mode on APOC will pop this one as well.

You could explain what "normal" difficulty is, I am assuming that it is balanced. Hopefully I am not voiding an achievement for a poor guide.

The achievement was originally called Normal. Hence the achievement stating complete the game on Normal. Subsequent updates, and the inclusion of Story difficulty means that it is now called 'Balanced'

i went into the menu to check out options, and i didn't even touch the difficulty slider, and the game warned me that i would be voiding difficulty achievements. It has and remains on balanced, but who knows.
