Mine uocked first play through. I didn't press B at all, including during Maniac Mansion. I also only used Y to do my inventory. Never touched the shoulder buttons.

Just a note that this achievement doesn't pop until the flag is raised and the credits start to roll. I was worried because I got "Yesterday's News" about 2 minutes before I got this achievement.

I pressed A in a few instances of dialogue and it did not affect my achievement. I never pressed B during dialogue, and I think that's the biggest factor in getting the achievement. Hopefully this achievement is not a buggy one.

I can confirm that skipping dialog doesn't void it. I skipped through almost everything on the second playthrough (pressing A on dialog). So it must have been skipping the intro in MM.

Well, it didn't unlock for me the first time. I'm sure I didn't skip a cutscene. So it must either be skipping some dialog wit Ben Franklin (You have to combine the kite and battery in open inventory, not using LB and RB to cycle through items. Otherwise you see the infinite fail dialog cycle, that's the one time I skipped the annoyance.) or pressing B on the intro in the Maniac Manson "minigame" ... So be warned, to do neither or you void the achievement for that run. :-(

If the achievement didn't unlock for you, you can use this video to do a speed run and get the achievement pretty fast. The person who uploaded the video, he skipped every cut scene possible so make sure that you DON'T skip when he skips but continue to speed through the conversations by pressing the A button. Avoid using the B button during cut scenes at all costs.

I skipped the “cutscenes” in the arcade game and still got the achievement. Those don’t count apparently, since they skip with a press not a hold.

A note on dialog: I was worried I had missed out on this on my first playthrough when I accidentally skipped a bit of dialog just after a small cutscene. I was already too far into the game to simply start over, so I just finished the game and crossed my fingers. Seems like I was panicking over nothing. You can totally skip dialog and it doesn't seem to count against this trophy.
