This is easy to do on Paradise Island. You only need to get to the end for it to unlock. Once you've progressed enough to see pinky on your compass go to her and say yes. A quest will pop up nearby that leads to an indoor hallway and once you reach the end the acheivment will unlock.

You should obtain this easily on the first level. I honestly don’t know if you can even die on it. I don’t remember taking any damage at all the first time through. Just move through the lasers on the side scrolling sections carefully and you will get this with ease.
