How to unlock the Romance Lydia achievement in Dead Age - Definitive Guide
On day 32 you'll be able to choose which love interest you want, for this achievement you need to choose Lydia. This game involves a lot of RNG, so it's possible you never meet her. To get her you need Hank to show up one day and give the quest Hank's Better Half. You'll find her when you finish this quest. You'll be given Date with Lydia next, and once this is done she'll join your camp. Then choose her on day 32 and successfully complete the game to get this achievement.

Lydia can show up first in camp, leading to acquiring Hank (i.e., you can get Lydia then Hank, or Hank then Lydia.
If you meet Lydia first, instead of giving a quest to find her husband Hank right away, she'll immediate ask you on a date. Go on this date and go along the Highway Day until you get to the diner scene. When you talk to Lydia this time, her husband will come up on conversation. Make sure you answer "Right" to continue the quest and head out to find Hank. Doing so, you'll once again unlock both Hank and Lydia's achievements at the same time.
NOTE: Because of the above possibility, you have more of a chance to have Lydia in your camp on Day 32 than Lyrissa or Jennifer (RNG Hank trigger or RNG Lydia trigger both lead you to have the opportunity to pick up Lydia), so my suggestion for those going for all the romance achievements is to do Jennifer or Larissa first if you have the option, as you're more likely to have Lydia in camp on a subsequent playthrough.

Make sure that on Day 32 you choose to spend your time with her Lydia (providing she is in your group), then when you finish the game this achievement should pop
During the course of a playthrough of Dead Age, you'll be able to get everyone in game to join your camp minus three people (so you can get max nine of twelve characters in one run). Of the missing three, two will be from the "random pool" (listed below) and one will either be James or Mary (when you do the mission to save one, the mission to save the other disappears.
The list of random characters is as follows: Bill, Hank, Lydia, Jamal, Jennifer, Larissa, Liz
Lydia is one of these random characters, so is not guaranteed to be available as a camp follower. Should he show up, you have three chances for that to happen the first week of the game (Day 1 - 7), and another early in week two (Day 7-14). After that, only one more "random" character will show up late in the game (around Day 28 - Day 32). Therein, if you're going for this achievement specifically, if you don't see the Lydia (or Hank) quest by Day 10 or so, restart your game.
Hank & Lydia
You can meet either Hank or Lydia first, and doing so will open a quest to unlock the other. We'll start with Hank first.
When you first meet Hank, he'll return to your camp, but you won't unlock the achievement for Hank yet. To do this, you'll have to do the "Hank's Better Half" quest you'll receive from him the next Day, to go searching for his wife. Do this quest (Highway Day) and you'll run into his wife Lydia. If you're interested in romancing Lydia, feel free to take the flirtatious options, as this weirdly doesn't affect Hank (their marriage is pretty bad apparently, and you need to flirt to unlock the romance option). At this point, both Hank and Lydia will join your camp.

(And again, feel free to flirt with Lydia in front of Hank, this doesn't affect him at all and allows for the romantic ending with Lydia).
Lydia Romance Option
Sometime later (after she becomes a camp member), Lydia will ask you out on a date. If you want the romance option with Lydia, accept. You'll meet Lydia outside of camp, then the two of you will head out along the Highway Day and have a romantic lunch at a diner (once you clear it of undead). There is no negative impact on Hank for doing this. Successfully completing this "date" will unlock the Lydia romance option.
Once you reach Day 33, you'll be given the option to romance Lydia (and possibly Jennifer and Larissa) or have no romance option at all, with each choice having a corresponding achievement tied to it. Since Lydia appears in camp when RNG selects either Hank or Lydia to be one of your RNG camp followers, its best to romance Larissa or Jennifer first if available, as Lydia is available more often and "no romance" is always an option.
The achievement itself will unlock on Day 42 upon reaching one of the "Successful" endings of the game (it won't unlock on one of the "Bad" endings).

You will get the choice with whom to build your love relationship on the 32nd game day. Select Lydia and complete the game with any positive ending to receive this trophy.
First, you need to agree to go on a date with her in a “cafe” the morning after Lydia arrives at the camp, and when she offers intimacy, reciprocate her proposal.
