Because of the end game collapse update, the hatch will always spawn when you are the last one alive, regardless of how many generators have been completed.
Solution:
This is across all games. There are two ways to escape. One is by powering five of the seven generators on the map, then opening one of the exit gates and leaving.The other is by going through the hatch.
The hatch will spawn randomly in the map when the number of generators done is 1 more than the number of survivors left. If all 4 people are still alive then it will spawn once the final generator is done. If 3 people are alive then their need to be four generators done, ie. 1 generator remaining. And so on.
The hatch will open when you are the last one left in the trial. So the other survivors must either escape or have been killed. You can wait for it to open and then jump in, or you can bring a purple or pink key, which will allow you to open the hatch at your discretion.
There are some places on some maps that the hatch tends to spawn at more than others for me. For example, in Badham Preschool it seems to love spawning in the basement of the school in the little corner where there is an old mattress on the floor and often a chest or totem spot nearby.

Got the achievement. If you are new to the game, my suggestion is, to level up(re-up) William Bill Overbeck to level 15.

The exit gates will open after 5 generators are repaired and the exit panel is powered. The hatch will appear on the map when one more generator is repaired than the amount of survivors still alive (4 alive = 5 generators repaired, 3 alive = 4 generators repaired, 2 alive = 3 generators repaired, 1 alive = 2 generators repaired). The hatch will not open unless you are the last survivor on the map, or a key is used to open it.
When you escape your 50th time, the trophy will unlock.
