How to unlock the Evil Incarnate achievement in Dead by Daylight - Definitive Guide
For this achievement you need to use Ultra Rare add-on Judith's Tombstone (Ability to kill Survivors in Healthy or Injure State). You can't finish it with hook sacrifices.
Firstly you need to increase the amount of Evil Within Tier III. You will basically need to stalk survivors (LT button) until they turn red to reach it. Then you pretty much have to find all 4 survivors and kill each one of them (with RT button). I recommend you also get the add-on Fragrant tuft of hair because that will make Evil within Tier III infinite.

nope..it does not work with ebony mori. You need add-on Judith's Tombstone.

I got this achievement legit and it is extremely nerve racking. Not only do you have very few chances with the add ons ( i would recommend both Judith's Tombstone and Fragrant tuft of hair) but if anyone disconnects you cant get it either.
There was a recommendation to watch how people load in and disconnect if they come in together but I have played many games and due to their typically being more survivors then killers my lobbies have almost always instantly and completely filled up. Not only that people might be more willing to quit if they are all playing separately. So on that count that advice may help, may hurt, but will definitely take more time to start a match.
Unfortunately, this like so many other things (if not boosted) takes a decent amount of luck.
I'd recommend putting on perks that stretch out the game as you will need to have a lot of stalking power built up to trigger endless tier 3. Ruin and thrill of the hunt so you know if they are working on them (good opportunity to stalk them) Sloppy butcher to slow down healing. Maybe Dying light to grind things to a halt once you eventually kill the obsession. Maybe BBQ if you are hooking them.
Long chases are not your friend. Keep them off the gens and your totems. If you can easily drop them do it as it takes more time up but if you can stalk and injure them and move on thats just as good. If you get a looper you can probably just completely stalk them instead of chasing and then leave them for another target.
If you get all but 1 (since you are instant killing them the last may have a chance at the hatch. You could slug the second to last one but I don't know if you can kill them on the ground (I would think it would count but am Unsure).
A worse killer rank might be another good choice. Means either deranking or waiting until after rank resets. Although if you wait to rank reset I would give it a few weeks so that all those low rank survivors get back down to the low ranks again.

I had only recently got this achievement, so I wanted to put down the methods that got me it. It does take some time, and if you happen to come across a survivor who hides in lockers purposely, finds a hatch, or hits the door immediately, sorry about that. I will list all perks, add-ons and the offering below. Remember, try to be highly alert with the perks below, and try to stalk from longer distances to prolong evil progression. Hope it works for you too !!
Perks:
Bitter Murmur - Each time a generator is repaired survivors within 16 meters are revealed for 5 seconds, with the last gen revealing for 10. Since you will be stalking from the off, having hints on where they are, and using aura's to identify survivor's locations is big.
Play with your food - If you do come across the obsession, letting them escape will increase movement speed. Without attacking you can gain 3 stacks (15%) which will negate our add-on penalty. Just to note 2 stacks is enough to surpass the add-on movement penalty.
Claustrophobia - It is preferential, but I found that I could catch survivors easier after gaining Evil within level 3. Normally with 2-3 gens being left after getting level 3, this will make any further gen completions more dangerous for survivors, and easier for you to catch up.
Insidious - I enjoyed the undetectable effect, however, I found most survivors saw me coming from a mile away. Choose an extra perk to fit your playstyle.
Add-ons:
Judith's Tombstone: Grants you the ability to kill at level 3. It will decrease movement speed by 9%. Play with your food stacks can easily negate this.
Flagrant Tuft of Hair: It may take 200% more of evil, but you will have unlimited kill ability for the rest of the game. For me once I gained level 3, I could focus on cutting survivors off into segments and getting the elimination.
Offering:
Vigo's Blueprint: For a low-value offering, this is what swung the last survivor my way. This will spawn the hatch most likely in the main building. You can use this to bait the last survivor into your sight lines. Once you see the survivor or hear them, shut the hatch and chase them down.
Perks:

The introduction of the End Game Collapse in some ways added some new pitfalls but in other ways you have more control and can make your own life easier.
To kill a survivor with tier three Evil Within you have to use the Ultra rare add-on "Judith's Tombstone" that allow you to perform an instant kill attack on standing survivors. This differs from all other "mori" kill attacks, which all require the survivor to be in the dying state before you can initiate the kill. All you need to do is get right up against a survivor for the "Kill" prompt to appear, then tap the basic attack button () but only tap it. If you hold it down you will instead make a lunging stab attack. Using Judith's Tombstone requires a substantial amount more of Evil to be stalked from survivors and then tier 3 still only lasts about a minute. To deal with the problem of survivors waiting out your tier 3, you'll need to utilize the other Ultra Rare Add-on "Fragrant Tuft of Hair." This makes tier 3 last for the remainder of the match allowing you all the time in the world to seek out victims. This leads us to the final important detail. This add-on also requires a tremendous amount more of evil to be drained to reach tier 3. Both Add-ons combined, you'll need to drain 3 survivors all the way to be able to get to Evil Within tier 3.
Essentially the strategy is to slow down the game as much as possible and stalk as much as you can as fast as you can to reach tier 3 before the survivors put too much pressure on you.There are generally a few problems you are going to encounter.
1) The biggest problem as mentioned, is that both add-ons require massive amounts of extra evil to reach tier 3. You'll need to heavily stalk all 4 survivors to get there. You'll need to be able to sneak up on survivors and get long stalks off of them before they notice you. For this the only perk I find helpful is The Doctor's "Monitor and Abuse" which represses your terror radius when not in a chase.
2) Second major issue is that Judith's Tombstone makes you move considerably slower, and this really hurts. The only true counter to the speed issue is The Shape's own perk "Play With Your Food". If you can find your Obsession early and gain Tokens on "Play With Your Food" you'll be mobile again and getting around will be a lot easier. And as long as you don't use your basic attack you wont lose tokens. The instant kill doesn't even count as an aggressive action and doesn't remove tokens either. This perk is pretty much required for this or you'll never be able to get around fast enough.
3) Third issue is the Escape Hatch. The hatch spawns when there's 1 more generator repaired than there are survivors remaining, or when the last survivor is remaining. Its actually beneficial now to let 3 gens get finished so when the match is down to 2 remaining survivors, you can find where the hatch spawned. After you kill survivor #3, head straight there and close it before the last survivor reaches it. At this stage, Survivors carrying keys are your worst enemy because they can just reopen the hatch and escape. If you notice a survivor is carrying a key, you'll want to kill them earlier in the match and hope no one else finds the key.
4) The end game collapse. After you slam the hatch shut, you'll have to find the last survivor before his 2 minute timer runs out or before he opens an exit gate and escapes. It's not required but I do strongly recommend using the Basic perk "Whispers". You should be able to see which door a survivor is hiding near and prevent them from opening the gate. If this is the route you choose to go, I also recommend pretending like you're heading away to get them to leave their hiding spot then doubling back to catch them just after they start trying to get the door open. This way is easier than searching for a hidden survivor.
5) This last problem is one you personally have no control over. You need to kill ALL 4 survivors in the match. The game now kicks you back to the start screen and refunds any add-ons/offerings if anyone disconnects before the match starts which is great since you can no longer load into a match with less than 4 survivors but that doesn't help against the rage quitters that will DC when things start to go bad for them. Almost all my failed attempts came from rage quitters that dropped out after the first kill. Just keep at it though and eventually you'll find a team that stays to the end of the match.
Since "Monitor and Abuse" and "Play With Your Food" are the only perks that are really required, you have a few options at your disposal to slow the match down or help track survivors. I leave this up to you because it really comes down to your comfort level with your own play style but here are some great options to run with:

Discordance (Legion Perk)
Corrupt Intervention (Plague perk)
Tinkerer (Hillbilly perk)
Hex: Ruin (Hag perk)
Whispers (Basic perk)

Discordance can show you where 2 survivors are working together which means more evil to stalk is gathered in one place. All the tips are nonsense, you need 2 red addons (a whole tombstone and a red lock of hair), save up 3 sadia, it will become endless and come close and kill
