I used the perks: No Mither, We'll Make It, Empathy and Urban Evasion.
The trick is to focus on unhooking players and run with them to heal them. and just watch the aura's of injured players and run towards them. if done good, you'll get a few healings per match.
At the moment it's a bit tedious cause a lot of killers use The Plague now and it's very hard to heal players.

My achievement is showing 100% but haven't unlocked, any way I can fix it?

Running perks that leave you broken and give you a boost will be very beneficial during this achivement.
Any survivor can earn this but it seems like it was designed with Jane's perk "Solidarity" in mind. You must heal other survivors while you yourself are injured. This is cumulative so you need to heal for the full equivalent of 50 total healing meters. It doesn't matter how many Health states were reached.
