There are two access points to "Guardian's Haven" in the game. The first area you can access it from is the "Cavern," the "Cavern" is deemed as the 'natural route.' The other access point is from "The Forgotten Sepulcher," you need two boss cells to activate this and have to have beaten the Giant boss at least once.
The "Guardian's Haven" is the area where you fight the Giant boss. All the details concerning everything in and about the room and boss fight can be found here brought to you by 'Gamepedia.com'

The achievement description hasn't been updated yet. The Guardian Haven is not available until beating The Hand Of The King for the 1st time. It unlocks when reaching the Throneroom for the 1st time from High Peak Castle.
After beating the final boss the first time, the Homonculus rune is rewarded, use this to cross the lava pit in the room behind the busted door in the Prison Quarters. It unlocks the door in the graveyard which leads to the cavern. This biome can be quite tough. Use of the 1st boss stem cell is not required to enter the Cavern and The Guardian Haven, so this can be done on normal.
Only the Steam version has the right description for this achievement at the moment, which also has achievements for reaching the Cavern ("You Dig"), the actual Guardian Haven ("Hot and Cold") for the 1st time and beating The Giant ("Size Doesn't Matter" ).
Maybe they will fix this in future updates for the Xbox One, Windows 10 and PS4 version.

So this achievement is for getting to the Throneroom for the first time.
