Failed Homunculus are in the Undying Shore, they appear dead at first until one of the Apostates revive them so if you rush through you may miss them. Undying Shore can be entered from the Fractured Shrines, Graveyard, or Stilt Village. To initially unlock the path from Fractured Shrines to Undying Shore, you have to wear the Cultist Outfit (use homunculus rune on a corpse or unlock the outfit). After the first time, the door remains unlocked permanently.
