This achievement is very simple.
First, you can access the Derelict Distillery in the passageway just before the High Peak Castle, after you have already accessed the Throne room in a previous run.
In there you'll notice big ennemies with barrels on their back, that's the infected workers. Try to find one alone (or kill ennemies nearby) and wait for him to launch a barrel at you.
All you have to do is use a regular sword on the barrel, it will turn blue, go back at the worker and kill him. The achievement should pop right away.

Just wanted to comment how I got this.
I was on the level and in a part one Infected Worker walks away from me and, in the same floor there was a Shield bearer. The last ones usually stop when they see you on the floor. The Infected Worker turned back facing me, threw a barrel and it exploded on the shield that the Shieldbearer was holding, dying.
