To unlock the Leghugger you must find the old lighthouse keeper found in the Stilt Village, which once killed will also drop a Crowned Key. While not useful for this achievement you will need it to access the Infested Shipwreck in order to get the rest of this DLC's achievements. Anyway, once you get the Leghugger it is a creature in which you summon to attack enemies for you. As quoted by the Dead Cells wiki: "After 100 bites, Leghugger will evolve into adult form".

Does he have to get 100 bites in one life?
When using Assist Mode, after a checkpoint he has to be summoned again. Does this reset the counter?

This feels like an annoying achievement because Legs isn't aggressive at all... he barely takes bites and just stays really close to you.

Edit: Still didn't confirm if it's needed in one reset or not but I got it fairly easily facing some Elites in Clocktower. Just summon Legs then jump on a ladder and stay there. Legs will keep attacking and the enemy mostly can't reach you.

The kills will carry through between areas assuming you dont die: I didn't get enough enemies in Clocktower to evolve it but i popped it out in Shipwreck and after around the 3rd kill it evolved

This is done at the 1st location after opening the weapon selection at the start. Freeze + Leggrab often drop together. We freeze and wait until he devours the enemy. 9-15 mobs and the pet evolves.
