How to unlock the Who's a good boi? achievement in Dead Cells - Definitive Guide
To get this achievement you have to beat The Hand of the King with an unsacrificed Mushroom Boi in your possession. "Sacrificing" means activating him a second time so he explodes. You are not allowed to sacrifice the Mushroom Boi at any time through your entire run once he is in your possession. This achievement cannot be earned in the same run as Bound for Hell or Pact with the Devil.
Apart from playing the game normally and being lucky to get the Mushroom Boi at some point and then remembering not to activate him twice, I was also lucky because I had been saving up my blueprints so that they don't clog my chests and rewards with crap. I still had the Mushroom Boi unlockable for 1 cell. So at the final Collector before Hand of the King I bought Mushroom Boi and only used him for the final fight, remembering not to double activate. Achievement popped when I detached my head to go back to the Prisoner's Quarters. It was only a bit easier because it meant the Mushroom Boi was at a high level for the fight which helped, and I didn't have to concentrate on keeping him the whole run (and I could easily see myself accidentally doing a double activation.)

You can also add that the achievement WILL NOT unlock, if you (like me) sacrificed the mashroom, walked past the Mama Tick and got the 2nd(new) mushroom from "funding" the blueprint. I beat the last boss with the 2nd mushroom in my inventory from collector and achievement didn't unlock

Just wanted to add to this - I was just able to obtain the "Who's a good boi?" achievement in the same run as obtaining both "Bound for Hell" and "Pact with the Devil" by finding a 2nd Mushroom Boi inside of Morass of the Banished.
I unlocked the blueprint for Mushroom Boi during the run on my way into Morass of the Banished and picked it up before heading in. Inside of Morass was a chest that contained another Mushroom Boi. I left that on the ground, and went through the rest of the level and sacrificed the original Boi to the Swamp Priest's altar, which removed the item from my inventory and unlocked the "Bound for Hell" achievement.
Then I went back and picked up the Mushroom Boi from the chest. When travelling to the Nest, Mama Tick allowed me to pass without the fight, unlocking the "Pact with the Devil" achievement. I continued on to the end of the run through Throne Room, keeping the new Mushroom Boi in my inventory the entire time without blowing it up, and after traveling through the fountain to return to the Prisoners' Quarters it unlocked "Who's a good boi?".
I believe since the original Mushroom Boi is removed from inventory upon sacrifice at the altar, it doesn't count as having sacrificed the new one that I picked up. Not sure if it would have worked if I had just blown up the original Mushroom Boi away from the altar and then picked up a different instance of the Mushroom Boi item - I had only activated the first one to sacrifice at the altar and at no other point.
This requires finding a 2nd Boi inside of Morass of the Banished though, which may or may not be likely.
