Just unlocked this today, so here's my two cents:
1. My first save file was from before any updates that I know of (December 2022).
2. I played through the game multiple times unlocking every other achievement. I also played through a couple extra time trying to grind levels and find the Unicorn.
3. Then I started a new save file (My name was 'Unicorn Hunt'.
4. I quit missions for 3 playthroughs to unlock the Unicorn as an available item.
5. Next playthrough, I grinded money until I unlocked the Industrial Area.
6. Once here I played 5 Ghost Jobs thru Day 29 (no Unicorn).
7. On the last day (Day 30) I played the Ghost Dimension (no Unicorn) and saved Beryl's soul.
8. On the next playthrough, I grinded money until I unlocked Restaurant Avenue.
9. Here, I played 4 Ghost Jobs, on one of these (before Industrial Area was unlocked) one of the civilian rooms had the Unicorn in a slime bubble. After I recovered from my shock, I cleared the room, bubble popped, and I picked up the Unicorn. Achievement popped after I activated the item (before I left the room).
Not sure if this is helpful at all, but I figured some more anecdotal evidence might help. Good luck everyone!

This is the worst achievement in the game by far. I hate luck based achievements that you have no control over. I’ve farmed the ghost dimension well over 20x and industrial state levels over 50x trying to get this last damn achievement.

You do need to have beaten the game at least 3 times, and all the sources I have seen describe it as dropping in 5 star industrial level and ghost dimension. Originally I had the verbal warning perk and would go through the ghost dimension entirely and proceed to quit out of the job right before killing the final boss in order to not end the game as I assumed this would be the easiest way. After my 5th loop I still needed a few achievements, including this one and completing certain jobs/killing assithaunt ghost. I went through all achievement sites among other things and found one on a playstation site since the chance of this item dropping is broken everywhere (This is my first guide so I am trying my best to cite this source). While it is no guarantee they found that higher item rarity would spawn the lower level a character would be, so they would intentionally quit out after hitting the appropriate drop areas to reset their level, and it seemed to work for them.
So here is what I would suggest if this is all you are missing and cannot get it to drop. Doing this method I did not even have to reset my cloud save like I originally thought as it only took 18 jobs to find one. Although this method does require patience and solid game skill because of the sponginess of enemies. It is also important to note that even with the potential increase in odds this item is still very rare (the game rarity is described as one of a kind/belongs in a museum), so it may still take some time to get the item. I just was not having luck with about 35-40 runs in the ghost dimension + 10ish Industrial levels at ghost level 5 using the verbal warning perk strategy.
- Play through the game normally doing things you may need for other achievements. Based on skill I would also suggest not taking the verbal warning perk because it would just waste one of your days, but if you need it to hit the industrial levels I would suggest it.
- Hit the point in the game where Industrial level 5 ghost difficulty is unlocked, which typically I got by playing 1-2 industrial levels that were 4 ghost difficulty after hitting $500,000. Start up a level and quit out/die to be demoted back to character level 1, and then it comes down to preference.
- You can either play through the loops normally and then restart the process if you were to be unlucky enough to not get the drop or there is a save trick. A good explanation of the trick exists on the achievement Undead with No Bloodshed by M16phenom. Essentially it involves turning your xbox into offline mode, but it will allow you to infinitely replay the days after you have set yourself up without having to get back to this point.
-The next thing I did was start up Industrial levels on 5 ghost difficulty and run around until I saw the unicorn, which looks like the achievement picture on this website. I only killed the ghosts if I was in a trap room, but I never leveled myself up in order to keep my character at level 1. It is tougher, but I noticed a difference in item drops as I was getting a lot of the items the game would put in the rarer categories or ones I had not seen. Like I said I did not even need to use my reset point while using this method.

This worked for me after just 10 jobs. I wish I had seen it much sooner, since I did 150+ jobs just looking for this after finishing all the other achievements. It's aggravating to play the 5 ghost job at level 1, but I think it really does work since I also found 4 other previously undiscovered Rare and "It Belongs In A Museum" items before the Unicorn.

I did something similar, I got to 1,000,000 to unlock the ghost dimension and ran the cloud save / offline trick. I got this on the first floor of the ghost dimension. I had died and started over to get rid of all my perks. I did level up several times in the level and I found the unicorn item trapped in slime. I was playing offline so I went online, beat the room, picked up the item and used it. Bloop.
