To unlock the ghost dimension you must earn $1,000,000. Then you must get through one floor from each district followed by a final boss without getting damaged. Every room you enter is a trap room. Upon entering every room, immediately take note of any items that will explode and damage you. Then either blow them up right away or avoid them completely. I would also recommend having at least the following perks before attempting this to make it a lot easier on you.
-Cool Operator III
-Rapid Fire III
-Multi-Shot II
-Paintrain V
-Poisonous Plasma II
-Shot Recycler
-Supercharged Vacuum
I also made sure to take the sheriffs badge and shotgun items with me to make quick work of the final boss.
Edit: If you got this game for free with gold, you must have your Xbox set as your home Xbox to use the below method.
In order to get unlimited attempts at this, get to a point where you have all the perks and items you want to start the run. (At this point I quit the game and reloaded it to make sure my console save and cloud save were synced). BEFORE starting the level, go offline. Once you are offline, start the level. If you get hit, quit out of the game to the dashboard and delete your save from the CONSOLE ONLY. Go back online. Start the game up again and your cloud save will load at the point before you started the level. Go back offline and repeat. Once you have completed it and are back at the main screen, go back online and the achievement will pop in a few moments. I used this method to try about five times or so before I finally got it.

In case you don't want to (or can't) do the save deletion trick there's a perk called Verbal Warning which gives you an extra attempt at this. It lets you keep your perks after dying. If you get hit simply die intentionally and give it a 2nd go.
Another helpful item would be the Marshmallow which acts like a shield against ranged projectiles. It's a common item so you'll probably run into one or two during your attempts.
Make sure to avoid all health items as you cannot use them at full health!

I went off-line and when I got to the second floor game quit out saying I needed to be online to play it
