Pretty simple and it doesn't really need a solution but if you are wondering what to do I'll tell you. So once you load up the game and begin playing (Make sure you're able to take control of your character and quit out by pressing

). Make sure to check that it says continue game now and that's it you're set to go and the timer starts. Either play the game and work on other achievements or hold off on it for 28 days, do whatever you want. Once it's been 28 days (Real days), load the game back up and the achievement will pop.
You don't actually have to play the game, just load the game up is enough for it to pop. If it doesn't pop, try loading the game up all the way to where you press Continue. Press Continue and that should do it. If that's still not enough, wait a couple days because maybe you counted wrong.
Also, this doesn't have to be 28 days without playing or 28 days in a row of playing.

The 28 days timer could've started at the time you started the game. So if you were on your 28th day but were on before the time you started it, maybe that's the reason why.
For example: You start the game on October 2nd at 4:00 PM. On October 30th, it won't become available until 4:00 PM.

Seems a bit glitchy. Started on 8 october, and at 20 november now. Still nothing.

Quick Method
First things first, You have to be able to play Dead Island Offline.
• Play the game While Online at least once
• Go to Settings, Network, Network Settings, Hit "Go Offline"
• Hard Reset Your Xbox One (Hold the Power Light/button for 5 seconds
• Go back to Settings, System, Time, Then change the date to a couple months ahead to be safe.
• Sign into Your account, Launch Dead Island D.E. , Get to the Play Screen with the bloody water, (i loaded up one of my saves to be safe)
• Then while the Game is still running, get back online (Settings, Network, Go Online,)
• Back to the game, then close it and re-open it, and you should get your achievement during the loading screen.

Worked for me, though it was a bit delayed for some reason. I re-launched the game, loaded my character, sat there for a bit with the game paused wondering why it didn't work. Then was about to try to find answers on Google or something when I just all of a sudden heard the achievement noise from my headphones. So you may just have to sit in the game for a bit if it doesn't pop right away.

This didn't work for me. The game won't load without being online, and going online corrects the date.

What I did to unlock this and it is still viable as of today 2022. This won't get you banned or whatsover so don't worry about it. 
Use an existing save-file, any while playing ONLINE. Then during your game session hit the
Options button. Go to OPTIONS, go to the tab ONLINE and switch the game type to "SINGLE PLAYER". Make sure to press 
to confirm it. Check again to make sure. Now quit the game from the menu hit 
. Now quit the game app entirely.
On your PS4 dashboard got to SETTINGS > NETWORK > UNCHECK "connect to internet" or simply disconnect your console from your internet by other means.
Now go to SETTINGS > DATE AND TIME > change the time, fast forward by a month or even more. Make sure you don't go backwards in time.
Then start up the game again while OFFLINE, still not connected to the internet and the trophy will unlock by the time you've reached the main menu! No need to start up any save-file.
After that feel free to connect your console again and synchronize your trophy list.
