Menu>Options>Controls - set "Fight Type" to "Analog"
By holding down the left trigger while moving the right analog stick, you may now control the direction in which you swing your melee weapon. For this achievement, you need to be using this combat setting.
Fast travel to the Arena Lobby and enter Arena A, as it is the easiest arena.
Here you must now survive 15 CONSECUTIVE waves of enemies, meaning that you cannot die at any point in between.
If you, like me, strongly dislike the analog combat controls, then I would suggest using kicks in the early waves and guns (all 3 types, full of ammunition) later on. This way you avoid actually using the specific controls.
If you choose to do this in co-op (which is recommended and generally much easier), your co-op partners DO NOT have to use the analog combat controls. As long as you use them, and do not die during the 15 waves, you should get the achievement. Also, you may let your co-op partners do the majority of the work, as long as you get at least 1 kill per wave.

I just unlocked this today, and in co-op you can die and it still keeps counting, just hope your friend stays alive long enough to either heal you or finish the wave
