This is story related and can't be missed.

You will be given the captain's locations in 3 different quadrants as main missions. Once you defeat their followers, they'll start running, but eventually they stop and you can kill them.

It popped for me the second time through the dlc. I took out all three captains in one go this time, the time that glitched I'd taken a break from playing before the last one.

Or it could have been Microsoft's achievement servers acting up on the first attempt.
