To achieve a "Perfect" bonus on a round, you must complete the round without dying once. This is not very difficult, and should come with playing the game naturally.

I think this achievement is glitching on me. I've done this MANY times but the achievement still hasn't popped. :-/

the same for me, got perfect also many times, but no achievement?! :-(
and the progress bar here on TA says that i got a perfect.

Weird, I died and the person I played with completed one round and this popped for me anyway.

Quite an easy achievement, you need to complete the round quickly and without dying.
