On the third chapter you will begin to lose your mind and fight against your mind just before being ported into the boss battle area, when you beat the cult leader after destroying 3 markers you pop out and he is there.
There are two achievements one to kill him one to spare him this is extremely simple to get both, do it and kill him once you kill him the achievement will pop simply reload checkpoint repeat the boss fight then leave him earning both.

you should really rephrase this. you don't fight carver. i thought i had a huge thing spoiled for me
there is a way to phrase this without spoilers. just say "when you have the choice to spare or kill the heretic, do so and so" - nobody knows who or what the heretic is anyway

Yes i'll give you that , at the time i couldn't really think of another clearer way to mention the area, however with mention of the cult leader and boss fight may still be enough detail people will understand. i will make a alteration.

This achievement is unlocked for sparing the Heretic Cult Leader during the later chapter in the Awakened subset.
Following a boss battle in Isaac's subconscious (you'll have to shoot down three markers, and then the cult leader himself) you will return to reality and see the cult leader on his knees, talking.
Simply run past him and continue the game as normal to unlock the achievement.

It can either be missed or gained but this achievement is simple to either miss (which gains you another) or to get.
For this achievement simply leave him alone and proceed into the elavator.
Basically near the end of the DLC story arc you have to fight the cult leader who has harassed you for ages and after a brief encounter with him in a hallucination you can either gun him down or leave him be.
During the fight simple kill his pals (DUH) and shoot him until a marker turns full on yellow then blow it up and he will get weak. Then shoot him until he explodes, repeat 3 times......its that easy :)
Video below shows the fight sequence and me making my choice.

Towards the end of chapter A3, the Cult Leader will be on the ground. You will have to walk past him without killing him and head to the elevator for the trophy. The trophy should unlock after you activate the elevator.
If you want to get the Heretic trophy (the opposite to True Believer) but have let the cult leader live you will have to replay the chapter.
