How to unlock the Final Regeneration achievement in Dead Space - Definitive Guide

This achievement is awarded for completing the “Premeditated Malpractice” side mission. I do not believe this mission is available until the very beginning of Chapter 11: Alternate Solutions, but it may be possible to backtrack to it earlier on if the tram stations are accessible and you can find your way back to Cryogenics.
Edit: Credit to DrShoe1976 who advise that the mission can be collected in Chapter 5. However, not all of your tram stations will be active at that point in the game.
At the start of Chapter 11, your RIG will want you to continue on with the game but instead, enter the door in the opposite direction named Cryogenics. You will return to the location where you had to freeze the Hunter (regenerative Necromorph) in the cryo-chamber. Move through one of the two doors and pick up the mission highlighted purple within the chamber.
All side missions can be tracked on the third tab of your RIG and selecting the next objective at each interval will help to direct you if you’re unsure (your directional line should be Orange and not Blue when selected).
- Head through Cryogenics and back to the Medical Deck, returning to the main hub where you broke through the barricade in Chapter 2. Now head into the Research Wing and use the elevator to access the tissue sample processor on the lower floor, just behind the door to the right. Process the tissue sample.
- Head back the way you came to the room with the destroyed barricade. Now enter the Emergency room and follow the route round to the Intensive Care Unit (ICU). Play the recording visual recording.
- Return to the room with the barricade and onto the tram, and head for the Mining/Engineering Deck. Enter Mining and take the lift down to Deck C. Progress though the floor and enter a room beyond the entrance of where you untethered the rock, called Ore Storage. Play the audio recording on the left of the room.
- Return back the way you came and take the tram to the Bridge. Go to the Main Atrium and play the visual recording a few paces from the door on the right side (just beyond the first two work stations).
- Return to the tram and go to Hydroponics. Head through to the West Grow Chamber and take the lift in the middle of the room to the second floor. Enter Hydroponics Control and then into the room off to the left named Nutrition Systems. The air is poisoned but it should not cause you any problems. Head through to the next room and play the audio log.
Side mission complete. If it was not possible to backtrack earlier on and you started from Chapter 11, simply return to the tram and head back to Hanger/Cargo/Tram Control to continue with the main story.

I started this in Chapter 5 - once the Hunter has been dealt with you can pick-up the Tissue Sample straightaway.

Once you reach the Diagnostics Lab in the back, listen to the final recording and unlock the achievement.

This achievement requires you to complete the new Premeditated Malpractice side-quest. This is a really cool little mission that confirms a theory on the origin of the Hunter that was only hinted at in prior games such as Dead Space Extraction. Once you begin the side-quest, it can be easily tracked and completed through the Missions tab of the RIG menu. It will even let you know if you have access to the area where you’ll find the next objective, or if you don’t.
All three of these new side quests can be completed at any point before the end of Chapter 11, when you hit the point of no return. You’ll be prompted at this point that you won’t be able to return to the USG Ishimura, so make sure that, before you accept this prompt, you head back into the Ishimura, create a save file, and complete all three side missions (see Whole Again (40G) and Full Clearance (30G) as well).
To start this side-quest, you’ll need to pick up the Hunter Tissue Sample from within Cryogenics - it’s on the floor in the central chamber where you froze the Hunter in the first place, and can be found immediately after freezing the near-immortal necromorph, right at the end of Chapter 5, as Chapter 6 begins. You can then track the rest of the mission through the Missions tab. This quest can be started as early as Chapter 6, and finished before the end of Chapter 11.
All three of these new side quests can be completed at any point before the end of Chapter 11, when you hit the point of no return.
