This achievement is for completing the entire campaign only using the Plasma Cutter. It is easily doable on medium difficulty and those who are familiar with Dead Space could do it on Impossible difficulty without any real issues.
However, there are a few tips to consider:

Use your power nodes solely for Suit and Plasma Cutter upgrades only, they’re no good on other weapons that you can’t use. There are enough Power Nodes in the game to max out the Plasma Cutter and your Suit on one playthrough if you take the time to collect them.
At the start of the game, grab yourself the HP Suit upgrade using a Node and then just focus on the Plasma Cutter upgrades - further Suit improvements are not necessary until Chapter 4 onwards. This also applies to impossible difficulty but feel free to be cautious, it’s better than taking an unnecessary risk.

- Guardians (stuck to the walls and spit out single, immobile lurkers). Don’t get close until dead!
Quite often there will be an explosive red canister or generator in the immediate vicinity that can dispatch them in one (not the Pregnants or Dividers when whole, only when they are broken into smaller enemies - using a canister on these whole only opens them up).
Overall, this achievement is not hard, but it is possible to make a mistake, therefore I would like to re-emphasise point 1, DO NOT even equip other weapons, put all unrelated items into the store and sell unwanted items to reduce the risk of a mistake.

Not sure what the parameters are for this achievement but seeing how there isn't any info out yet, I'll just state that I accidentally picked up some other guns during the campaign, but paused and backed out to the menu during the animation and loaded into a checkpoint before picking it up. Still got the achievement just fine for any of those that may have worries with it happening.
(I also didn't pick up any upgrades for the other guns but I think that was me being overkill and not wanting to rage if it somehow voided the achievement 😅)

only using might mean kills. if you only kill things with the plasma cutter it might count, even if you picked up other weapons but they stayed in your inventory the entire game

As per our Achievement Guide & Roadmap, we recommend doing this on Story or Easy difficulty, either as your first playthrough or as part of your New Game+ run, which should make things pretty easy.

Using cutters from the DLC does not reset the trophy, successfully completing the game with the Super Powerful Plasma Cutter will give you the trophy.
