This achievement is for completing the entire campaign only using the Plasma Cutter. It is easily doable on medium difficulty and those who are familiar with Dead Space could do it on Impossible difficulty without any real issues.
However, there are a few tips to consider:
You can pick up other guns throughout your playthough, just be sure NOT to fire them. To be safe, don’t even equip them - return any other weapons and upgrades to the safe to avoid any mistakes and free up space in your inventory.
Use of Stasis, Kinesis and Melee does not affect your playthrough.
Make best use of your inventory. Only stock Medkits, Plasma ammo and a Stasis pack or two. Sell the rest back to the store for credits. The game is intuitive and will only tend to drop ammo for your equipped weapons, reducing pressure on purchasing ammo. But, if you want to farm credits, it’s worth keeping the Force Gun equipped whilst remembering not to use it (probably best not to do this).
Use your power nodes solely for Suit and Plasma Cutter upgrades only, they’re no good on other weapons that you can’t use. There are enough Power Nodes in the game to max out the Plasma Cutter and your Suit on one playthrough if you take the time to collect them.
At the start of the game, grab yourself the HP Suit upgrade using a Node and then just focus on the Plasma Cutter upgrades - further Suit improvements are not necessary until Chapter 4 onwards. This also applies to impossible difficulty but feel free to be cautious, it’s better than taking an unnecessary risk.
That said - don’t neglect your Suit, especially later in the game. Upgrade it accordingly so that you can continue with its HP upgrades, but air and other parts are not essential.
Be sure to find the 3 Plasma Cutter upgrades because having it maxed out will make the final levels easier:
- Chapter 2: Intensive Care - Ammo Capacity (after destroying the barricade, it is in the Clinic).
- Chapter 3: Collision Course - Heat Pack (it’s available in the store for 11K credits from this point on).
- Chapter 8: Search and Rescue - Weighted Blades (in the maintenance room just before you take on the Leviathan Remnant).
Edit (November 2023): Credit to Zack R T who advised that the first and second upgrades have been rotated. I have no idea why MOTIVE did that but I can confirm following a recent play though. Guide amended.
Importantly, know your enemies. The Plasma Cutter can deal with almost anything but 4 enemy types can cause you issues, mostly because they’re a drain on your ammo but they can catch you out. These are:
- Swarms (the creeps that jump your back).
- Pregnants (the ones that drop Swarms if you shoot them in the Torso).
- Dividers (the ones that break down into heads or other individual body parts).
- Guardians (stuck to the walls and spit out single, immobile lurkers). Don’t get close until dead!
Quite often there will be an explosive red canister or generator in the immediate vicinity that can dispatch them in one (not the Pregnants or Dividers when whole, only when they are broken into smaller enemies - using a canister on these whole only opens them up).
Overall, this achievement is not hard, but it is possible to make a mistake, therefore I would like to re-emphasise point 1, DO NOT even equip other weapons, put all unrelated items into the store and sell unwanted items to reduce the risk of a mistake.
Edit: Just to Clarify, I am not sure if this can be done on a New Game Plus, as I did it on my first playthrough. I would be surprised if you couldn’t though, providing you do not use any other weapon. If anyone would be so kind to comment and advise, I’ll update accordingly. Thank you.

Not sure what the parameters are for this achievement but seeing how there isn't any info out yet, I'll just state that I accidentally picked up some other guns during the campaign, but paused and backed out to the menu during the animation and loaded into a checkpoint before picking it up. Still got the achievement just fine for any of those that may have worries with it happening.
(I also didn't pick up any upgrades for the other guns but I think that was me being overkill and not wanting to rage if it somehow voided the achievement 😅)

only using might mean kills. if you only kill things with the plasma cutter it might count, even if you picked up other weapons but they stayed in your inventory the entire game

This trophy is nowhere near as daunting as it sounds; veterans of the franchise know how powerful this thing can get and new players may be pleasantly surprised.
You get the Plasma Cutter early in Chapter 1, almost immediately after you get attacked on the Ishimura.
Like all weapons in Dead Space it has an alternate mode however, unlike the other weapons, this is a simple switch between horizontal or vertical shooting. Personally I use horizontal at all times, aiming for the legs first then laying waste to whatever is still moving but both serve their purpose well enough.
Since you can't use any other weapons any others you pick up should stay in your inventory until you can put it in storage. You can upgrade the other weapons if you wish but you can't fire them (some people say you can't KILL with the weapon and that may be true but why risk it?) and really you shouldn't even bother upgrading them unless your suit and Cutter are maxed out.

This is not as hard as it sounds. Since you will only be using the Plasma Cutter you can sell all of the other guns' ammo and put the credits towards the Plasma Cutter. Just be precise with your shots and keep your cool.
Having DLC weapons will not negate this trophy, however using them will. The Heavy Damage Plasma Cutter DLC will not affect this trophy whether or not you use it.

As per our Achievement Guide & Roadmap, we recommend doing this on Story or Easy difficulty, either as your first playthrough or as part of your New Game+ run, which should make things pretty easy.

This can be done in a good 7-8 hour playthrough on easy. Make sure you upgrade the Plasma Cutter all the way. It will help a lot. Stock up on nothing but ammo for the Plasma Cutter and a few med packs. Sell everything else. If you run out of ammo in a boss fight, I hate to break it to you, you're screwed.

Using cutters from the DLC does not reset the trophy, successfully completing the game with the Super Powerful Plasma Cutter will give you the trophy.
