You get the axe in Act 1 Scene 4. This trophy can be a bit tricky as you can't aim with the axe.
You can't push enemies on the ground with your feet and kill them with the axe; it will not count. The best tip is when a shadow runs at you, hit 
at the right moment and you will eventually hit his head. You can also try to hold when you fight a group of shadows.
