Note that there are no health upgrades on Nightmare mode, so you must do this during your first playthrough.

This is a health upgrade and there is a total of 3 in the game you can pick up any of them to unlock the achievement, but this is the first 1 you come to.
You should get this naturally, but if you don't load up Act I Scene 5. Before going right (which is the way to are meant to go) go left. Now use your axe on the boards and crawl through the gap. Keep going left and there will be a dead body, pick up the health pack from the body.
Note: There are no health upgrades in Nightmare mode, so you must do this during your first playthrough.
