Go to the Milk Barn and buy the map from Keith ($2000), he will then tell you a story and mark a location on your map. Head to Cope's Tunnel and enter the portal to the "Other World", clear each room of enemies before going through the door until you reach the final room with the Submachine gun sat on the table at the rear.
