I did this after the cave cutscene with Fragile, right at the start of the game after running downhill a little way.
Hold
to open the tool selection wheel and select 'pee' on the left of that wheel.
Now follow the on screen button prompts to aim and let it flow into the cave!

Do you know why i only get one mushroom and other players have a stack on the spot? Not that it matters but i want to do it 😂

Most Players Get This On Accident. Norman Reedus' Bladder Gets Full And You Got To Pee. Your Shame Is Marked By A Mushroom Hologram And The Achievement Pops.
If You Stop After Each Delivery To Use The Facilities And Take Showers (To Make Free Grenades) You Won't Get This One. Took Me 70 Hours To Finally Need To Pee Outside.
Side Note -
I Think Drinking The Energy Drinks Also Increases Your Bladder Meter.
