Probably best way to achieve this is to "disable" reactor.
There is a schematic inside the building that will tell you order in which you need to cut the wires on reactor itself.
It's red/blue/green/yellow
Then you can go guns blazing even on Fia

Just a note, the sequence is different for each player, mine was Blue/Ref/Green/Yellow, solution is the same, just incase someone goes in blind without looking at the schematic in their game

To get this you need to kill Fia without the reactor going off. Simply put the best way (imo) to do this would be to do this stealthily, as long as Fia herself doesn’t notice you before you kill her your good. (I was detected by some of the enemies in the reactor room but took them out and I was still able to get this.)

Fia is located in Fristad Rock and she spawns during Noon (time of day)
This shouldn't really be difficult at all. You can go in guns blazing or just take it easy. Taking it easy would be the best option if you haven't played for too long since you won't have much health.
To make it even more easier, before going into the bunker you'll see 2 snipers on top. Climb up (watch the mines) and there will be a turret on top of that bunker. Hack it and take it with you downstairs. Don't use it yet. Kill all enemies first that are inside the bunker.
When you reach the area where Fia is, place the turret somewhere close to a wall or door opening and eventually the turret will detect 1 enemy and start killing it. More enemies will get lured by the sound of the killing and hopefully your turret will kill them all.
Fia should now be alone. Get inside and obliterate her.
If you somehow do mess up, you got enough time to kill her before she blows up everything.

Although the previous guide to disabling to reactor before killing Fia is relevant and works, it’s not entirely necessary.
You don’t have to disable the reactor to gain the Trophy. Killing Fia without her being alerted to your presence will suffice. So a stealth approach works just fine i.e - Machete/Aether/Shift abilities.

You can find Fia in Fristad Rock during Noon. Fia is the trickiest Visionary to kill, because as soon as you are detected by her or her Eternalists, she will activate a nuclear reaction that will kill you and the entire island. To get this trophy, you have to kill her without activating the nuclear reaction.

- Hard way: Try to sneak into her room and kill her by stealth kill or as quick as possible

Easy way: Deactivate the reactor before attacking her. This will, however, include a little bit of stealth action, because you need to enter the reactor room without being spotted. The easy way to reach the reactor is the following:
- Enter the bunker from the top (beware of mines and tripwires in the road)
- Once inside, take the battery from the field nullifier near the broken elevator and jump down.
- Use the battery to unlock a door to your right and hack the camera on your left (watch out for the Eternalist in the area!)
- Enter the door under the camera and go to the end of the room, where you will find a scheme with 4 colored wires and numbers near them. Take note of them, as they will not be recorded yet.
- Go back to the door you opened with the battery and go for a panel that you can open by kicking it. It will lead you to some vents that can be deactivated by pushing a button.
- You are now in the reactor area. Hack the antenna and cut the wires following the numbers of the scheme (the log will now update). You can be spotted while doing this last step, as cutting the wires will stop the reactor.
- Now, kill Fia

I was having a lot of trouble with this, so I went to Complex early morning and switched the power over to Fristad (you have to have advanced far enough in the story to unlock this quest - which isn't actually related to this achievement but helped me).
Switching the power allows you to access a secret door behind Fia's warehouse, letting you bypass A LOT of the eternalists that are lurking in the main entry way. I think I had to deal with only 4 eternalists on my way to Fia and they were easy to stealth kill.
Once I got to Fia's office she was distracted, and I was able to stealth kill her as well.

Fia has wired a reactor so that it will explode and wipe out Fristad rock (and anyone in it) if she dies. To get this achievement you need to shutdown the reactor prior to killing her. This is a bit easier than it sounds. Once you penetrate into her fortress (there are multiple entrances), the reactor is on the ground floor of the central area (Fia is in a room above it). While there are a number of guards around, you can work your way to the back side of the reactor (more or less under Fia's room) through a variety of paths. Once there you can disable it with your hacking machine.
Once it's disabled, you need to kill Fia to unlock the achievement. It will unlock immediately upon her death, even if you don't manage to escape.

In order to get this achievement, you basically need to stealth your way through Fia’s compound in Fristad Rock at noon. This is actually a little tricky, because if you get seen in the bunker, and other Eternalists are alerted, well, so is Fia who sets off a countdown, which can end the loop.
What you need for this is: a silenced pistol (or any silenced weapon, if you can) and Shift. The rest is trial and error. If you can initiate the Visionary lead “Afternoon Delight” as well, that would be help.
First things first, exit the tunnel, shoot the four Eternalists – two straight ahead, two by the booth (without alerting anyone, of course). You then want to head right, behind the large structure, and make your way through the trucks and into the compound. Few things to note: there are a load of mines and trip wires here, you can work out where they are and avoid them; or you can shoot them (which does alert enemies, but won’t set off the reactor explosion). Our strat was to snipe the two guys sat on high above the bunker, and then Shift our way across from the top of one truck to another. We then went in through the top door and down the elevator shaft.
While inside you can silenced kill the first six or so people (in pairs) in the main structure. Just stay close to cover and only shoot when you have a guaranteed headshot. The ideal route into Fia’s chambers is taking the left path under the camera (which you hack - on the walkway that leads into the main room) and going in via the showers.
After taking the left-hand path, you end up in the room where the code for your Blackreef delivery machines are (it’s on the white board). If you shoot the glass on the right, and Shift across the corridor, you’ll end up in the showers. There are two guards here. One who stands with their back to you on the left, and the other who is sat down and can be headshot with ease.
Next, you need to get through the compound into her quarters. I found baiting enemies into the showers was the best solution as there are more than a few here. It's worth bearing in mind that yes, the exclamation marks can go red, but if they do you have to kill them quick enough so Fia isn’t alarmed.
When the top route is clear, make your way through to the back (hacking the camera as you turn the corner at the far wall). That back corridor basically has two doors that lead to Fia's quarters and her office. She's in a room in-between those two rooms.
When in the room adjacent to hers, it’s key to use quick door peeks (and a down tag if you can) and listen to the sound of her steps. She basically walks around the room. If you can time it right you can wander into the room and hit right bumper and take her down with ease. If you do this correctly, you'll get the achievement.
While you’re here, it’s worth doing “Afternoon Delight” as well (AKA Dangerous Liaison (30G)), which is why we mentioned it earlier. Check the achievement guide for that achievement for more insight on how to complete that.
