On the loadout screen, equip:
4 Character trinkets
3 Guns (purple or orange tier) with weapon trinkets in all 3 slots of each gun
2 Slabs with 2 upgrades each
You'll likely have to wait until later in your play-through because of the slab upgrades. The fastest way to unlock this after having all slots filled is to go to Karl's Bay in the morning and kill Harriet. Don't pick up any new weapons or slabs, exit though the tunnels and the achievement should unlock.
To make killing Harriet much easier set your game to "Single Player mode" and turn on "Infinite Reprise" and "One Shot Kills" in the options menu under "Gameplay".

This one is bugged for me, done these requirements multiple times and nothing. Anyone else having issues with this one?

Finally unlocked this when I finished a mission with full loadout killing only the visionary in the level. Don't know if it's required, just that this popped it for me. I had mix of reward and normal guns.

As the achievement says, you need a full loadout with no empty slots, which means you need: two Slabs (with two upgrades equipped), three weapons with three trinkets each, and four character trinkets. Set that up, kill a Visionary, escape, and this is yours.
Couple of quick notes: this isn’t possible with the HALPS Prototype as you can’t equip trinkets; and finally: Egor at night in The Complex is probably the easiest kill (if you’ve not set up Charlie and Fia, that is).

To get this trophy, simply equip everything you can. You need:
- Three weapons with three trinket slots (DON'T USE weapons without slots or less than 3)
- Two slabs with both upgrades
- all 4 personal trinkets
Now, select the district you prefer and kill the Visionary within it.

In the current version of the game, there are no problems with weapons from the deluxe version, checked personally.
