When looking at a mission, the hazard level is the yellow "warning" icon with the exclamation point in the center (not to be confused with the footprint shaped "Length" indicator or the lightning bolt sign "Complexity" indicator). Just to clarify what JohnnyInterfnk mentioned above: you can play a Hazard Level 3 mission and complete it, then play X number of Hazard 2 or lower missions (succeeding or failing them) and then complete the other 2 Hazard Level 3 missions. I had a gap where I did some Hazard Level 2 missions thrown in and my achievement tracker still progressed once I came back and played the Hazard Level 3 missions and succeeded.

I hosted and my other two teammates got this and I didn't. Bah. Tried a hard reboot. Hopefully it's just delayed.
