Act 4 consists of the following Missions.

Lend a Hand
Strange Bedfellows
Cut Off the Head
Seize the Day, Control the Future
Into the Heart of Darkness

Into the Heart of Darkness involves you clearing the path to the Golden Gate Bridge. Once you defeat all of the resistance there, including the cut scene against the final threat at the bridge, return to Shondus Consulate. Turn the mission in, watch the cut scene, and this achievement will pop.
