Most likely you will get it automatically while you deal with the mechanics. If you die, you lose all collected D-tokens. They remain in your tombstone, they can be collected by finding it and shooting it during your next descent into the dungeon. If you don't take it, it will disappear and the coins will be lost. If you see that you have no lives, then it is better not to risk it and use a magic rope to return to the city. To do this, click and select the appropriate menu item, the rope costs a penny and pays for itself in a few seconds, but its use will allow you to save a lot of nerves if you suddenly die with 100,000 coins one step away from the last boss.
