Not all that difficult as its a slow QTE, Points wise you need around 6500-7000 for the S-Rank.
My first completion was an S at 8500+ as you'll get a large bonus for finishing it all (under 60secs completion of all 4 gourds) may take a couple of tries if your timing is off/mashing too slow.
