How to unlock the Challenge Accepted achievement in Destiny 2 - Definitive Guide
As you progress through the story you will unite the 3 Vanguard members (Zavala, Cayde-6, and Ikora) and they will join all the other vendors at the Farm and will be told to speak to them.
Upon speaking to Ikora you will unlock challenges which will require you to do certain tasks such as getting multi-kills or ability kills during activities such as Strikes & Patrol. These can be checked during the activity by pressing and will appear on the right-hand side of the screen. Occasionally if there is something else being displayed on the screen there may be a delay so be patient if they don't appear straight away.
There are 3 unique challenges per activity/area and can be stacked amongst each character so this can be done within a day assuming you have at least two characters at the same stage (confirmed by OniAegis in comments below).
List of activities/locations confirmed so far for which the challenges differ:
• Earth
• IO
• Nessus
• Titan
• Strike
• Nighftall Strike
• Prestige Nightfall Strike (same challenges as normal strike but stack)
• Crucible - Quickplay
• Crucible - Competitive
• Crucible - Trials of the Nine
• Raid
• Prestige Raid*
*Currently unsure if there will be challenges in the prestige raid (which I'm assuming is likely).

I've now added a list with all the different activities and locations which contain different challenges (basically all of them) so it currently requires a min of 2 days to get the achievement unless someone can confirm they do stack between characters.

Can confirm this is account based rather than character based. Did a bunch of challenges on my first character and I popped the achievement while playing as a second character I created today and only just got access to challenges a couple hours ago.

All of the information in this guide is correct as of Season 15, Season of the Lost. I have finished my time with Destiny 2. If there are any changes to this achievement in a future update it will be up to someone else to write the guide.
With the Forsaken update what was considered "Challenges" were changed to "Bounties" and no longer work for this achievement.

Challenges have changed once again with the Beyond Light update. I logged in and my character was at 1100 light level.
There are several weekly challenges that you can complete each week. There are weekly challenges for doing bounties for NPCs in the Tower. There are also weekly challenges for doing Crucible, Gambit, Strikes and Nightfall Strikes, and dungeons. You will know there is a weekly challenge when there is a gold star by the option on the Destinations tab.
When you highlight the NPC on the Tower map in the directory it will show the gold "weekly challenge" under their description. Go to that NPC and you will be able to purchase 4 bounties that have specific requirements with your glimmer. The bounties only last one day after purchasing them.
The NPCs I was able to find in the Tower were
Zavala (strike bounties)
Banshee-44 (gun bounties for various weapons
Lord Shaxx (crucible bounties)
The Drifter (Gambit bounties)
Suraya Hawthorne (clan bounties - mainly for raids)
You can purchase a 5th bounty from the NPCs for more glimmer than the normal ones that gives a random requirement.
Just doing what the bounties requires does not automatically complete it. To claim the bounty reward you just open the directory and go to the Quests tab. From there all the bounties you have aquiried will be on the right side of the screen. The ones you have completed will be at the top. Click on them to claim the rewards and have them count for the weekly challenge.
For the playlist items you only need to do however many matches it requires. Generally Gambit and Crucible want 3 matches each. Nightfall Strikes have percentage bar, where harder Nightfall Strikes fill the bar faster. Once you have completed the required number of matches the weekly challenge automatically completes.

Man, I've been doing all the "daily" and weekly challenges every week for at least 6 weeks now. Has anybody unlocked this since they changed to this format? I swear I've done way over 30 by now. I really wish the progress was tracked on this one.

You may need to do more than 30 challenges; it didn't pop for me until I did 37. My 37th challenge was for completing Crucible bounties, and it popped after I talked to Shaxx and claimed my reward. You may need to do the switch profile trick.
The exact challenges offered can vary week-to-week, but the following is a list of frequently-appearing challenges, what vendor/activity offers them, and how to complete them. I've read that some of these challenges won't appear until you've hit the soft power level cap, which is currently 1200.

Challenges are the new name for Destiny's bounty system, and they work in much the same way.
