How to unlock the Eye for an Eye achievement in Destiny - Definitive Guide

After completing
DestinyThe Young WolfThe The Young Wolf achievement in Destiny worth 53 pointsComplete the "Rise of Iron" quest.
and talking to some people in the rise of iron "town" area, you will unlock the "Bad Blood" Story mission in the plaguelands. Completing that mission unlocks this strike.

There are some hard jumps, but nothing dangerous in the strike until you reach the boss. You'll come to a shaft with an chained ogre at the bottom. Interacting with it will release it and summon Kovik.

The ogre is invulnerable and has one melee attack. He will generally follow one player around and slam at the that player. His attack can be dodged by jumping. One member of your stirke team should shoot the ogre, and then focus on holding his aggro and dodging.

Kovik has a big gun that will fire nonstop, and can kill players quickly. He also has a melee stomp attack that knocks players back and also inflicts lots of damage. All players should remain in cover, peeking out to shoot Kovik in the head. However, be careful, as Kovik can teleport.

Once you've completed the strike, return to the vanguard scout in the rise of iron "town" area to turn in the quest and get the achievement.

Am I missing something? I’ve now completed the Kovik’s Sin quest with two different characters and still no achievement.

Did you return to the vanguard scout to report the quest?

Turns out it was just a very delayed unlock. Unlocked while I was out and my Xbox was off.

This trophy unlocks after completing the Wretched Eye strike.

This strike unlocks after the Rise of Iron story has been completed. Here's a video of the strike itself, it's pretty straight forward.

The Kovik's Sin quest starts from Shiro-4 in the Iron Temple and will eventually lead to a new strike (The Wretched Eye) added for this content. Follow the quest steps to unlock the achievement when you return to Shiro.

"Kovik's Sin" can be started after completing the "Rise of Iron" quest. There are two steps to it. The first is to complete the "Bad Blood" mission. The second is to complete the strike "The Wretched Eye."
