You do not have to be unseen, although being seen will cost you a Ghost XP bonus. You just need anyone who sees you not to have an opportunity to set off an alarm. Some areas have no alarm whatsoever, so you cannot fail there. Some alarms are scripted and guaranteed to happen as the story progresses. These will not count against you either.

More of a tip than an overall solution...
I attempted this on 4 separate playthroughs, and the key to getting Foxiest of the Hounds unlocked was in the first boss fight (Barrett). What finally worked was rushing him right from the start with the Stun Gun and tazing him "to death". I read somewhere that he often destroys the cameras in the room with grenades, which sets off an alarm that counts against you.
