This achievement is simple, just play through everything the game has to offer
Here is the List :-
Human Difficulty
Demon Hunter Difficulty
Son of Sparda Difficulty
Legendary Dark Knight Difficulty
Dante Must Die Difficulty
Heaven or Hell Difficulty
Hell or Hell Difficulty
Ranking nor order count of completions count. You can D Rank everything and start from Dante Must Die (although not possible) and still get this achievement.

You can start at dmd mode if you buy the unlock all mode thing for 5$.
