It appears that you need to beat Mission 19 and Mission 20 in succession without using a continue to get the achievement to pop. The 1st time I played thru I used a continue on the 1st battle (Mission 19) but not on the 2nd (Mission 20) so I figured that just beating Mission 19 again without using a continue would get me the achievement but it didn't pop until I beat Mission 20 again right after Mission 19. Just skip all the cutscenes after the last battle (Mission 20) and you'll get it.
