This is pretty easy, you just need to sneak up behind an enemy and press and hold B until they are dead. Do it 20 times and you have the achievement.

Thanks was wondering what I was doing wrong, the games says to press B I didn't realise I had to hold it redface

Takedowns are performed from behind most enemies throughout the game. Approach an enemy from behind and press to perform a takedown. You will not be able to take down enemies in power armor or some of the most elite units, but you should get to 20 takedowns before the missions where those types of enemies are prominent.

To perform a Takedown, you need to sneak up on the enemy from behind and press “o”. The trophy is simple and easy to do at the beginning of the game, because... There are plenty of simple opponents.
