This achievement can be very easy to miss, especially if you are just learning the ropes of the game. If you are like me and did not bother investing in the "Endurance" tree, you are not completely screwed if you are going to go for a "completionist run."

A good majority of your augments are purchaseable at Dr. Niles's clinic. To start off, you will need Cerebrospinal Fluid to unlock a slot. A second augmentation that you can collect, Sy-nterface, also unlocks a slot, which can be found while exploring the game. With those two items installed, you can install two mods of your own choosing.

Why did I say "two of your own choosing?" If you are going for a 100% completion run, you will need the Enhanced Artificial limbs for performing high jumps and the Respirator implant in order to complete one of Holms's (The drug store owner) side missions.

Now, if you could only fill four augmentation slots with the items I mentioned, you have two "grace items" to fill the gap. One grace item will be the "Dead Eye" augment, which can be collected after finishing the "Good Cop, Cyber Cop" side-mission, which can make it "augmentation #6." Last, but not least, if you are completely out of options after doing ALL of the steps listed above, your last augmentation will be filled by completing the "Finding Soto" achievement. As soon as you get your "enhanced artificial limbs" upgraded, the achievement will pop up.
